Job Summary:

Shriners Hospitals for Children- Portland is seeking an on-call Social Worker to join our Care Management team! This position requires understanding of common ethical and legal issues in social work practice in health care settings including biopsychosocial-spiritual assessments, use of the strengths perspective, client and family engagement in all aspects of social work intervention, and case management/ care management/ care coordination/ health care navigation.

Responsibilities Include:

Discharge and transition planning

Client concordance with and adherence to the plan of care

Advanced care planning, palliative care, including pain and symptom management, hospice and end-of-life care

Identification of child abuse, trauma, neglect, and exploitation

Crisis intervention

Facilitation of benefits and resource acquisition to assist clients and families, including an understanding of related policies, eligibility requirements, and financial and legal issues

Advocacy with other members of the interdisciplinary team and within the health care institution to promote clients' and families' decision making and quality of life

Client, family, interdisciplinary, and community education

Addresses family systems issues, including the impact of health care concerns, illness, and disease on family relationships

Knowledge, Skills & Abilities:

Master's Degree in Social Work is required

3 years' experience as a Social Worker is required, 5-7 years preferred, experience in a Pediatric setting is preferred

Current Social Work License in the State of Oregon

Case Management Certification (CCM, ACM, or ANCC) is required or obtained within 6 months of hire or eligibility

Experience conducting and documenting psychosocial assessments and interventions to meet physical, functional, social, emotional, educational needs of the patient/caregivers is required

Experience in a healthcare setting with community and national resource navigation and coordination, payment methodologies, mitigating psychosocial barriers to patient/ family adherence is preferred

Demonstrates strong communication skills - both oral and written

Excellent customer service and service recovery skills is required

Demonstrates desire for and potential for growth and professional development; The ability to manage conflicting priorities is required

Demonstrates empathy, caring and compassion for children with complex medical conditions and their support systems

Proficient in Microsoft Office (Word, Excel, PowerPoint and Outlook)

Demonstrates willingness to support efforts to maximize the efficiency and effectiveness of the electronic medical record

Bilingual in English and one additional language is preferred

VIEW JOBS 3/26/2020 12:00:00 AM 2020-06-24T00:00 Providence is calling an On-Call Behavioral Health Social Worker (LCSW) (0.0 FTE, Rotating/Variable) to support Providence Portland and Providence St. Vincent Hospitals We are seeking a Behavioral Licensed Clinical Social Worker (LCSW) that will independently provide comprehensive biopsychosocial/spiritual assessment, diagnosis and/or treatment/interventions/advocacy of patients and their support systems within the inpatient hospital setting. The LCSW incorporate concepts and theories associated with life span development, neurobiology, and behavioral change. The LCSW will collaborate with the patient and support system to develop and implement evidence-informed person-centered care plans across the care continuum in collaboration with the Inter/Multi-Disciplinary Team (IDT). The LCSW shall perform duties in a manner that respects patient self-determination, that reduces health disparities/barriers to care, and adheres to the NASW code of ethics, Standards of Practice (SOP), and mission. In this position, you will have the following responsibilities: * Independently provide strength based comprehensive biopsychosocial, emotional, spiritual, and cultural assessments. Assess for and recognize social determinants for wellness and recovery. Conduct ongoing assessment of patients appropriate to age and cultural background in accordance with patient directed goals. May include advance care and end of life discussion as appropriate to setting. * In the context of a clinical relationship, provide social work interventions to support and empower patients and their family/support systems to effectively cope with the impact of medical and/or mental health conditions, optimize functioning and improve overall well-being. Formulate, implement, and monitor a social work plan of care that supports and identify therapeutic solutions or adjustments to medical, social, emotional and financial problems which may impact health or well-being. Care plans are patient-centered, strength-based, and culturally-relevant. Utilize counseling, motivational interviewing, crisis intervention, education, case management, etc. as appropriate and required to the clinical setting. * As appropriate and required to the clinical setting, perform diagnostic evaluation of psychiatric and substance use disorders. Establish a diagnosis based upon DSM and ICD diagnostic criteria. * Incorporate evidence based theories and methods, provide specialized clinical treatment services as appropriate and required to the clinical setting. Utilize the Professional Edition: Current Procedural Terminology (CPT) for psychiatric and substance use disorders to resolve and/or reduce signs and symptoms of diagnosis as appropriate and required to the clinical setting. * Provide effective case management including triaging patient needs, adapting approaches to meet additional needs, and collaborating with internal and external resources as appropriate. * Create strength-based, collaborative care planning with the patient and their family/support systems. Develop and maintain effective working relationships with various departments, community health, social service agencies, payors and other partners. Advocate for support appropriate to the age, biopsychosocial needs, cultural, spiritual, and health literacy of the patient. Identify barriers to continuing care and planning of next level of care. Function within the existing care team model. Collaborate with the inter/multi-disciplinary team facilitating care conferences for complex patient care coordination. * Maintain timely, written documentation as well as verbal handoffs within professional standards and in accordance with department policies and procedures. Submit relevant ICD/CPT and DSM codes for reimbursement as appropriate and required to the clinical setting. * Act as an advocate for the needs and interests of the patient and their family/support system by supporting self-determination and autonomy. Advocate for navigation of complex healthcare systems, various payers, community services, education, system changes, etc. Adhere to mandatory reporting of abuse, neglect, and/or exploitation. * Provide clear and concise communication through consultation, education, facilitation and team organization to patients and their family/support system. Collaborate with internal and external partners, and patient caregivers to enhance provision of care and services in care settings across the continuum. Communicate changes in patient condition or goals of care related to medical, mental, social, spiritual, environmental, and eligibility factors to the inter/multidisciplinary team. Contribute to and update the inter/multi disciplinary care plan. * Perform duties in a manner that adheres to social work professional standards and code of ethics and in compliance with regulatory bodies; NASW (National Assoc. Social Workers), TJC (The Joint Commission), etc. Protect confidentiality and establishes professional boundaries. Maintain a constant awareness of safety and legal aspects of the position. Educate and communicate with the inter/multi-disciplinary team. * Seek and utilize appropriate consultation and supervision. * May provide qualified CSWA clinical supervision as eligible and as defined by the applicable Oregon Administrative Rule (OAR). Clinical supervision is an interactional professional relationship between a clinical supervisor and a Social Worker that provides evaluation and direction over the supervisee's practice of clinical Social Work. Promote continued development of the Social Worker(s) knowledge, skills and abilities to engage in the practice of clinical Social Work in an ethical and competent manner. * Provides social work, case management, and home visiting nurse services to high risk maternity patients across the continuum of care. Social workers are assigned to cover outpatient clinics and the hospital maternity and neonatal intensive care units. The Providence Beginnings social worker, in conjunction with the patient and caregivers, develops, implements, and monitors a plan of care to support positive birth outcomes and successful transition to parenting.
