POSITION SUMMARY

The Social Worker, a member of the multidisciplinary team, assists in patient care cases identified as difficult or problematic with regard to the physical, social financial or discharge issues of the patient and/or family.

ESSENTIAL FUNCTIONS

  • Identifies patients at risk or requiring assistance of the social worker through referral from the interdisciplinary team or interdisciplinary patient care conferences.

  • Communicates both verbally and in writing with the interdisciplinary team, the social, financial or discharge needs of the patient and/or family.

  • Collaborates with the interdisciplinary team to develop a plan of care and discharge plan.

  • Appropriately identifies difficult/problematic cases for discussion with the Director and interdisciplinary patient care conferences.

  • Identifies and utilizes appropriate community resources to meet patient discharge and continuity of care needs.

  • Participates in department quality improvement activities by identifying improvement needs, collecting data, and implementing plan.

  • Develops and facilitates support groups.

  • Identifies, assesses, and intervenes with patients and families needing psycho-social intervention

EDUCATION REQUIRED

  • Masters in Social Work (LMSW).

LICENSURE/CERTIFICATION REQUIRED

  • Current certificate of LMSW in the State of Texas.

EXPERIENCE REQUIRED

  • 3-5 years experience in acute care setting with experience in discharge planning and case management.

SKILLS, KNOWLEDGE, AND ABILITIES PREFERRED

  • Reads and writes using basic English grammar, bilingual skills English/Spanish.

  • Ability to listen, empathize and respond to patient, family, internal and external partners needs.

  • Basic computer skills.

  • Proficient skills in collecting and retrieving data from medial records and other information sources.

  • Effectively applies data retrieved from the medical record to the review process.

  • Document in a uniform and legible manner.

POSITION SUMMARY The RN Staff Nurse provides direct nursing care in accordance with established policies and procedures of Del Sol Medical Center and the Texas Board of Nurse Examiners. In collaboration with the patient, the patient's family, and other members of the Health Care Team, the clinical RN assesses, plans, implements, evaluates and coordinates treatment plans. ESSENTIAL FUNCTION * Clinical Role Model - Demonstrates an advanced level of clinical skills, rehabilitation nursing skills, and the ability to support others to achieve the delivery of quality care. * Leadership - Demonstrates interpersonal skills and competency in leadership, supervision and management of staff. * Administrative - Demonstrates excellent skills in planning, organizing and managing personnel resources in a cost effective and productive manner for the operational area of the nursing unit. * Customer Service - Demonstrates awareness of his/her unique position and method of dealing with customers. * Continuously strives to exceed customer's needs. * Successfully passes an annual Functional Independence Measure credentialing examination with a score above 80%.
