Broughton Hospital is currently accepting applications for a Social Worker to join our team. The position assists in obtaining information from admission throughout hospitalization to assist the Clinical Social Worker and Treatment Team in determining the best course of treatment. The Social Worker develops discharge plans through coordination of community resources and collaboration with the patient, Treatment Team, Clinical Social Worker, Managing Care Organizations (MCO), and other agencies. These functions are under the oversight of a Licensed Clinical Social Worker (LCSW). This position will manage a caseload of psychiatric patients, some of whom may be dually diagnosed. The Social Worker provides case management services and psychoeducation aimed at skill building for the patient.

Responsibilities include, but are not limited to, the following:

  • Assisting with the completion of 24-hour intake documents assigned to social work;

  • Identifying the linkage to social services and resources based on patients' needs while considering clinical assessments/recommendations and natural supports;

  • Identifying appropriate private or residential placement resources;

  • Completing residential placement referral applications and arranging pre-discharge site visits and appointments; including completing applications and visiting facilities or service providers to obtain services and placement;

  • Referring those with housing needs to TCLI and coordinating with In-Reach activities;

  • Working within the multidisciplinary team and taking part in associated meetings;

  • Providing written updates to assigned unit social worker and supervisor regarding progress or barriers toward goals;

  • Contacting patients, advising them about available resources and arranging appointments;

  • Performing direct functions, such as psychoeducation, maintaining documentation in patient records, or arranging meetings and aftercare services;

  • Collecting information and preparing reports concerning patient progress;

  • Updating the treatment team regarding resource development and discharge efforts;

  • Tracking and documenting the outcome of placement inquiries in the patient record to demonstrate consistent discharge planning efforts; and

  • Serving as a liaison with social and health service providers, legal and/or housing resources, and others, in the coordination and development of aftercare services/resources.

The normal work schedule is Monday through Friday, 8:00 a.m. to 5:00 p.m. However, flexibility is required for training purposes and to meet hospital needs.

 *This position is designated Mandatory and must report or stay over during adverse weather events.

Salary Grade: NC11

Recruitment Range: $42,464 - $74,313

  • Experience providing case management

  • Working knowledge of social work principles, theory base methods, and policy

  • Demonstrated ability work effectively with the interdisciplinary treatment team and/or other various departments

Management prefers individuals with:

  • Experience working with persons with mental illness, substance use, or intellectual or development disabilities

  • Experience working in an in-patient mental health facility

  • Experience creating discharge plans

Master's degree in social work from an appropriately accredited institution; or

Bachelor's degree in social work from an appropriately accredited institution and one year of directly related experience; or

Master's degree in a human services field from an appropriately accredited institution and one year of directly related experience; or

Bachelor's degree in a human services field from an appropriately accredited institution and two years of directly related experience; or
