Job Description:

The Social Worker 6-A is domiciled in the regional office of LDH/ Office of Public Health (OPH) Region 6 and serves as the Regional Social Worker. The SW-6A provides regional training and multi-disciplinary collaboration with other employees as needed.

The SW 6-A provides advanced-level social work services for the Children's Special Healthcare Services-Clinical Services Program which includes behavioral health assessments, assisting patients/families to cope with psychosocial stressors or chronic illness in a clinical setting through individual, family, supportive and or educational counseling.  The SW 6-A provides resources and/or referrals as well as Care Coordination and Transition services for CSHS-Clinical Services. The SW 6-A responds to referrals from OPH nurses to provide behavioral health assessments, consultation, information and resources to individuals of all ages.

Respond to other public/private settings to provide information and resources to individuals of all ages as needed.

The SW 6-A serves in advisory/training capacities to other agencies, such as the Head Start programs in the region.  The SW 6-A supervises and/or provides social service activities in the Behavioral Health Section to the Medical Special Needs Shelters as needed.

Availability for assignment to the Medical Special Needs Shelter/Behavioral Health Section to provide social work services during a disaster is mandatory. Usage of a personal automobile for work related travel is required.  Maintaining licensure as a Licensed Clinical Social Worker.

ESSENTIAL FUNCTIONS:

60%    Provides advanced-level social work services for Children's Special Healthcare Services-Clinical Services such as: behavioral health assessments completed in the EHR, diagnoses, individual and/or family therapy, if needed.   Compiling correspondence containing medical information and releasing it to other agencies and/or community organizations when approved by the patients.  Developing and implementing complex individualized treatment plans to address identified needs, problems, and behavioral or emotional reactions of patients and families. Provides care coordination and transition services to transition aged youth. Serves as advocate for the patient in resolving problems related to their needs, financial, medication, transportation, home, health, school issues, and referral services.  Directs the patient to available community resources by assisting them with the application process for obtaining benefits through the Department of Children and Family Services, Medicaid, Central Louisiana human Services District, Vocational Rehabilitation, and other state and private agencies.   Provides education and training to families and patients regarding Prenatal Care, Adolescent Pregnancy, Alternative Family Planning Choices, Sexually Transmitted Disease, Parenting, Child Abuse and Neglect, etc. and regarding their ability to cope with the daily pressures and problems associated with their risk factor.   Functions as a member of the health care team, which may include nurses, physicians, school personnel, etc.   Participates in staffing, staff meetings, and program evaluation and case presentations.  Provides clarification to the parents of team recommendations regarding treatment and/or service plans.  Attend weekly, monthly and otherwise mandated or suggested meetings with the Regional and CORE staff internally.  Participate in meetings with Statewide Social Work Consultant.
20%    Serve as the Regional Social Worker for OPH Region 6.   Provide and oversee the direction/performance of Public Health social work services in Region 6 in collaboration with the regional Administrator/Medical Director, and other regional staff as needed.  Review social work activities to assure quality and compliance with all applicable social work policies and procedures as well as adherence to the Code of SW Ethics and standards.  Interpret LDH/OPH policies and procedures for staff and clients as needed.  Participate in interviews, orientation, training of new employees regarding the role of the social worker in LDH/OPH and regarding the impact of relationships, and behavioral, emotional, social, and cultural factors on the course of illness and recovery process, in conjunction with the Regional Administrator/Medical Director, upon request.  Design and participate in continuing education programs for staff.  Respond to referrals for social work services from OPH agency staff to provide consultation, information and resources to individuals of all ages as needed or assigned.   Respond to other public/private settings to provide information and resources to individuals of all ages as needed.   Attend appropriate professional seminars, trainings, workshops, and other continued education programs as required or approved in order to maintain professional social work licensure and to enhance professional expertise/performance in the area of social work to ensure compliance with the Louisiana State Board of Social Work Examiners.   
10%    Provides social work consultation/input on multi-disciplinary teams within and outside of the agency and serves as the LDH/OPH representative for social services on various boards, committees, coalitions, etc. locally, regionally and statewide if assigned.  Provides support services to other programs within OPH.  Participate in meetings with the Families Helping Families Agency, health fairs and distribution of health promotion materials.  Keeps the Regional Administrator/Medical Director abreast of ongoing functions, activities, planning, and opportunities for collaboration.
5%      Participates in emergency/disaster planning and training throughout the year and providing assigned and appropriate services in times of disaster particularly in the MSNS.   Availability for assignment to the Medical Special Needs Shelter/Behavioral Health Section to provide social work services during a disaster is mandatory.  
5%      Performance of other duties as assigned by Regional Administrator or other individuals in a Supervisory or an Authoritative position over Regional Social Worker.

MINIMUM QUALIFICATIONS:

A master's degree in social work from an accredited school of social work followed by five years of professional level social services experience.

NECESSARY SPECIAL REQUIREMENT:

Positions require possession of Licensure as a Licensed Master's Social Worker (LMSW) or Licensure as a Licensed Clinical Social Worker (LCSW) by the Louisiana State Board of Social Work Examiners.

NOTE:

The master's degree in social work must be from a school accredited by the Council on Social Work Education, 1701 Duke Street, Suite 200, Alexandria, VA 22314.
