Overview

• All positions are located in Fresno/Clovis CA*

If you want to be a part of a team that operates like a family and challenges each other to constantly improve and grow together, you will succeed here. In this role, you will be responsible for providing psychosocial-emotional support and services to patients and families.

At any given time, you will be working with 35-45 patients, meaning that you will always have the opportunity to learn and encounter a variety of patients. Individuals on this team make up half of the case management team, a group of 129 members. Each floor of our facility has 1-3 Social Worker's on hand at all times, this means you will always be encountering a variety of specialties and types of care. As a member of our team, you will serve as a point of contact in order to assist patients and family in understanding, coping and adapting to the person, emotion and bio-psychosocial issues.

Responsible for providing psychosocial-emotional support and services to patients and families.

Assists patient and family in understanding, coping and adapting to the personal, emotional and bio-psychosocial issues associated to the patient's illness.

Provides comprehensive assessments and referrals, crisis intervention and care management. Collaborates with multi-disciplinary team and other internal and external personnel to promote an awareness of environmental and psychosocial needs and to develop and implement treatment plans that support patient-centered plan of care for both individual patients and the medical community. May provide functional direction to unlicensed social work staff and social work interns.

Qualifications

Education:• Master's Degree in Social Work (MSW) required Experience• 1 year of clinical, case management, social work or related experience required Preferred:• Case management experience in healthcare/social services agency preferred• Experience in an inpatient or outpatient care setting preferred Licenses and Certifications• BLS - Current Basic Life Support (BLS) for Healthcare Providers by American Heart Association (AHA) required Preferred:• ACSW - State of California Associate Clinical Social Worker preferred• AMFT - State of California Associate Marriage and Family Therapist preferred• CISM - Critical Incident Stress Management preferred• ACM - Accredited Case Management Certification preferred• OSW - Oncology Social Worker preferred• CHP-SW - Certified Hospice and Palliative Care Social Worker preferred• FCDBH-5150 - Fresno County 5150 Cert preferred• CCSP - State of California Children's Services Paneling preferred within 180 days of hire or transfer into position
