Your day.

As a Social Work Care Manager you need to know how to:

  • Assess for appropriateness of level of care setting from admission through discharge.

  • Identify patients requiring case management and takes the lead as case manager for those requiring intervention, especially with socially complex cases.

  • Interview patients and families to assess current functional status and support system.

  • Collaborate with the health care team to develop a plan of care by participating in patient care rounds.

  • Assure the discharge plan is in alignment with providing choice, assuring safety, and is timely and cost effective.

  • Assure the plan of care is individualized for patients with a chronic disease to support patient/family self-management following discharge.

  • Assist with scheduling discharges, pre-planning discharges and identifying discharge goals during patient care rounds to assist with patient flow on the units

  • Provide education to other departments/floors regarding care management services and support, discharge planning and plan of care.

  • Serve as a support to nursing partners in the area of social services, Medicaid applications, disability eligibility, ETOH resources and homeless resources.

  • Attend and participate in department meetings and participates in multidisciplinary groups as necessary and/or requested.

Your experience.

  • Graduate of an accredited school of social work, sociology or related field.

  • Social Work degree or related experience in case management or related field

  • Knowledge and experience with managed care

  • 2 years of case management experience, preferable acute care preferred

  • MSW or Master's Degree in related field preferred

  • LCSW preferred

Position Summary This is a part-time, non-benefited position of approximately 20 hours per week. The Program Coordinator for the MSW@Denver Program acts as a support person and liaison between section faculty, course developers, MSW@Denver leadership and other faculty support team members to ensure the quality delivery of all MSW@Denver courses. The Coordinator monitors course development progress and training schedules, works closely with all faculty teaching in the MSW@Denver program to ensure compliance with DU policies and procedures, assists the course lead in orienting new instructors to course content and live session planning, acts as a resource to faculty regarding student supports and coordinates the teaching evaluations and live session reviews. This position reports to the Associate Director of MSW@Denver. This is a non-benefited position not to exceed 1,000 hours in a calendar year. Essential Functions * Maintain schedules of course development and required training activities. Requests development and training payments in coordination with the GSSW Business Office. * Communicates with MSW@Denver course faculty regarding university processes and policies. * Coordinates evaluation and review activities. * Assists with orientation for new instructors. * Supports course leads in managing and updating content, communicating with section faculty, disseminating information, and communicating expectations clearly. * Answer or refer instructor questions as needed, providing quality customer service.
