Job Description:

The Nurse/LCSW Care Manager works collaboratively with physicians and other members of the health care team to improve the health of patients with chronic conditions or complex needs. This position educates patients and families to help them manage their health care needs. The incumbent facilitates communication, coordinates service, addresses barriers, and promotes optimal allocation of resources while balancing clinical quality and cost management.

Accountable for management of care management staff, including one or more of the following disciplines: RN and Social Worker Care Managers, Care Management Assistants, Care Guides, Health Advocates. Care Management staff are decentralized and provide specialty services to patients across the continuum

As a Manager Care Management you need to:

  • Manages the day-to-day operations of the department. Collaborates with the department management team in planning, program development, human resource management, budget planning, customer satisfaction, patient safety, communication, and compliance with regulatory agencies.

  • Coordinates the care and services for patients identified as needing assistance or meeting Care Management criteria. Collects in-depth information about a patient's medical, functional, and social condition, to identify individual needs in order to develop a plan to meet those needs. Works with patient and family/caregivers to determine specific goals and actions based on assessment. Coordinates discharge planning. Executes specific interventions to meet established goals. Organizes, integrates, and coordinates the necessary resources to accomplish the goals and plan.

  • Assesses the patient's prior level of functioning, access to and/or use of community resources and available support systems. Assists the care team in developing a plan of care which includes but is not limited to assuring appropriateness of services and care setting, assuring individualized support and education, determining the need for continued services, planning for discharge, and identifying and connecting patients/families with available community resources if needed. Collaborates with Physicians and other members of the health care team on the patient's behalf.

  • Identifies appropriate admission and continued stay issues. Enhances the quality of patient care through effective and efficient use of resources. Helps to identify strategies for reducing length of stay and appropriate utilization of services. Utilizes criteria including clinical pathway data and implements strategies to resolve controllable variances.

  • Attends, facilitates, and participates in rounds and case conferences. Advocates for patient rights. Monitors efficiency and availability of services and evaluates outcomes through variance tracking and data collection.

  • Identifies needs, facilitates, or provides education to physicians and ancillary departments/nursing units regarding case management, discharge planning process, and roles. Participates in multidisciplinary groups and development of guidelines.

  • Evaluates service needs and volumes and adjusts staffing levels accordingly.

  • Maintains effective and appropriate staffing and staff scheduling to ensure safe, quality patient care. Provides communication to associates and physicians through rounding, huddles, staff meetings and other forms of communication to relay updates regarding department updates, organizational activities, financial performance, educational opportunities, interdepartmental activities, and QI activities.

  • Responsible for problem solving to address issues relating to patient safety, care, and service. Promotes effective working relations and works effectively as part of a department/unit team and interdepartmentally to facilitate that department's ability to meet its goals and objective. Ensures coordination of care with other departments to promote the highest level of patient care and satisfaction. Participates in interdisciplinary committees throughout the Page 2 of 4 organization.

  • Manages human resource functions such as interviewing, selection, orientation, education/training, feedback, performance evaluation, and policy and procedure development. Maintains personnel files, including documentation of appropriate staff orientation, competency, training, discipline, and performance reviews. Oversees department timekeeping and payroll.

  • Collaborates with HR concerning employee issues and performance. Conducts employee coaching, counseling, disciplinary actions, and annual appraisals. Monitors staff adherence to hospital and departmental policies and procedures.

  • Participates in the preparation of operational and capital budgets. Participates in fiscal accountability of services, operational and capital planning, annual operating plans, business plans for new services and human resource structure.

  • Prepares and presents business plans, reports, and other statistical data related to the department.

  • Monitors supply inventories and capital equipment purchases.

  • Partners with the department director and leadership team to identify and drive quality and performance improvement initiatives. Participates in establishing mechanisms to design, measure, maintain, and improve the performance and quality of department services.

  • Ensures compliance with applicable regulatory guidelines and established departmental policies and procedures, objectives, quality assurance program, safety, environmental and infection control standards.

  • Participates with management team and staff in development of action plans in line with the organization's strategic initiatives and goals.

Minimum Qualifications

  • Bachelor's degree in nursing, Social Work, or a related field from accredited institution. Degree will be verified.

  • BSN, MSW or LCSW is Required- Current Valid State RN License is Required

  • Five (5) years of case management experience in a hospital setting required-

Preferred Qualifications

  • Three (3) years of supervisor or management level experience in a hospital setting is preferred.

Physical Requirements:

  • Ongoing need for employees to see and read information, labels, assess patient needs, operate monitors, identify equipment and supplies.

  • Frequent interactions with patient care providers, patients, and visitors that require employee to verbally communicate as well as hear and understand spoken information, alarms, needs, and issues quickly and accurately, particularly during emergency situations.

  • Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy. This includes frequent computer use and typing for documenting patient care, accessing needed information, medication preparation, etc.

  • May be expected to stand in a stationary position for an extended period.

The hourly range for this position is listed below. Actual hourly rate dependent upon experience.

$39.15 - $71.80
