Providence is calling for a Social Services Transition Associate (Per Diem, Days) to the Care Management team at Providence Regional Medical Center Everett in Everett, WA.

The Transition Associate will work as part of the Care Management team to perform a variety of patient centered administrative tasks to support patients and families within their specific discharge plan of care. We are seeking a dedicated team member who is able to work on a per diem basis that may include some weekend support.

In this position you will:

  • Work closely with patients and their families to help plan their post-discharge care

  • Act as a Liaison between outside agencies including Admission Coordinators for SNF's, RN's from Assisted Living Facilities, Home Health Agencies and Infusion Services, to coordinate and support discharge planning services

  • Handle discharge planning packets and insurance verification and authorization

  • Maintain Electronic Medical Records for discharge plans, insurance authorization information and other appropriate clinical information

Required qualifications for this position include:

  • Two (2) year college Associate Degree

  • Experience providing customer service to patients and families

  • Experience providing administrative support to professionals

  • Experience working with insurance payers

  • Knowledge of Medical Terminology

Preferred qualifications for this position include:

  • Educational courses in healthcare

  • MS Office Certification

  • Health care training

  • Knowledge of human anatomy & physiology and disease processes

  • Experience using MS Word and Excel software programs

VIEW JOBS 4/12/2019 12:00:00 AM 2019-07-11T00:00 Overview Incumbent is responsible for providing needs based assessments for clients in the Jail Transitions and Non Medicaid programs. This includes meeting Jail Transition clients at the local Snohomish County correctional facility. Incumbent may also provide psychotherapeutic services to meet the needs of certain program individuals. Position is Part Time, Flex. (20-37.5 hours per week). Position will work 20 up to 37.5 hours per week; hours are based on caseload. Hours may start out around 20 and as caseload builds, could work up to 37.5 hours per week. Starting Salary/Hourly Rate: (Position is Exempt, but paid hourly) $22.95 - $24.10/hr. DOE, plus excellent holiday / vacation / sick pay and health benefits! For internal candidates/union positions only: Compensation is set by agreed to language in the Collective Bargaining Agreement between CCSNW & SEIU 1199NW and determined based on experience. Responsibilities MAJOR DUTIES AND RESPONSIBILITIES: Therapeutic Services: * Provides a needs-based assessment. * Assesses and implements treatment goals and measurable objectives with client. * Coordinates/participates in information exchange with other care providers including the case management team. * Implements and ensures ongoing appropriate treatment. * Maintains ongoing documentation for clinical records within agency time-lines and according to WAC mandates. * Provides a comprehensive diagnostic assessment that is strength based for each client. * Works with client and case management team to formulate a service plan with achievable goals and measurable objectives. * Completes all documentation within an established time frame and according to WAC requirements. Meetings: * Meet with the supervisor as needed for the purpose of case and diagnostic formulation. Access to and protection of PHI: * Responsible for upholding a culture of privacy and security in highly confidential work environment (HIPAA covered entity service area). * Complies with all CCSWW policies and procedures that involve access to and safeguarding of client Protected Health Information (all forms). Other work related duties as assigned. Qualifications MINIMUM: * MA in psychology, social work or related field. * Holds MHP Status. * Able and willing to obtain WA State Agency Affiliated Counselor Certification OR currently have WA State Licensure. * Reliable transportation, current WA State Driver's License & proof of current automobile insurance. * Support and uphold the mission, beliefs and values of Catholic Community Services and Catholic Housing Services. * Support and contribute to a creative, collaborative and respectful environment that promotes teamwork. * Demonstrates the necessary attitudes, knowledge and skills to deliver culturally competent services and work effectively in multi-cultural situations. * Criminal History Background Checks are required prior to employment. PREFERRED: * Licensure (LMHC or LSW) with Washington Dept. of Health. * Masters level clinician with at least two years' work history with homeless and/or jail related clientele.
