Social Services Manager Description


The following statements are intended to describe the general nature and level of work being performed and are not intended to be a complete list of all responsibilities, duties and skills required.

Essential Duties & Responsibilities

  • Recruits, hires, provides orientation/training, and retains a sufficient number of qualified staff to carry out the responsibilities of the Social Services department. Ensures employee performance meets or exceeds expectations and periodic performance appraisals are conducted on a timely basis.

  • Collects and assesses data relevant to patients' psychosocial needs, risk factors for psychosocial deterioration and responses to interventions.

  • Implements social service interventions that achieve treatment goals, address resident needs, link social supports, physical care and physical environment to enhance quality of life.

  • Completes required forms and documents in accordance with company policy and state and/or federal regulations.

  • Coordinates discharge planning.

  • Facilitates advanced directive decision-making process.

  • Coordinates Family council.

  • Coordinates grievance complaint process.

  • Performs other duties as assigned.

  • This position requires the supervision or one or more Social Worker and a Master's Degree in Social Work or in a human services field (such as Sociology, Special Education Rehabilitation Counseling, Psychology).

  • One year supervised social work experience working directly with individuals in a health care setting.

  • Current Social Work license in the state, if applicable.

Physical Demands & Environment

May be required to respond to critical issues on a 24/7 basis. Working in normal office environment, employee may be required to sit, bend, stoop, use keyboard, see, talk and hear. May occasionally lift medium to heavy objects including assisting with the movement of Residents. Identifies the social, emotional and economic needs of patients and families through a continuous assessment process. 2. Participates in the development and revision of a plan of care focusing on those needs that can be met through social work skills. 3. Implements the plan of care in accordance with objectives mutually established with the patient, family, physician and other members of the interdisciplinary team. 4. Provides individual and group counseling services or crisis intervention for patients and families to assist them in coping with their social, emotional and economic needs. 5. Evaluates the effectiveness of the social work plan of care and makes revisions according to patient/family response and social work judgment. 6. Consults and coordinates with other members of the interdisciplinary team regarding the social, psychosocial, emotional and economic needs of the patient/family. 7. Identifies and utilizes appropriate community resources and assesses patient/family ability to access them. 8. Advocates for the patient/family in securing resources from health care providers. 9. Communicates observations and evaluations effectively through reports, records and team meetings. 10. Evaluates patient/family response to interventions when referred to community agencies and satisfaction of the services provided. 11. Assists in discharge planning for hospice patient/family when hospice service is no longer appropriate or patient/family needs change or when evaluation for long term care is indicated and assesses ability to accept level of care change. 12. As a member of the interdisciplinary team, assists team members in understanding the significant social and emotional factors related to care of the terminally ill patient and the family and the social/mental stress that exacerbates the symptoms related to terminal illness. 13. Participates in the orientation, training and coordination of volunteers working in the hospice program as appropriate. 14. Participates in orientation and in-service training programs for hospice staff. 15. Participates in agency and community programs as requested to promote professional growth and understanding of hospice care. 16. Maintains a high level of competency, growth and development, including ensuring license and certifications are current and in good standing at all times. 17. Follows guidelines as indicated in employee handbook. 18. Documents accurately and appropriately on the laptop, uploading by midnight. 19. Participate in QA/OBQI initiatives with problem-solving via action items. 20. Assesses the patient/family psychosocial status, potential for risk of suicide and/or abuse or neglect. 21. Performs bereavement risk assessment within five days of admission. 22. Initiates advanced care planningfor all patients. 23. Assists with funeral planning issues and transfer of responsibilities regarding fiscal, legal, and health care decisions when appropriate. 24. Participates in the on call rotation as scheduled. 25. Assesses environmental resources and obstacles to maintaining safety. 26. Identifies family dynamics and communication patterns. 27. Evaluates patient/family response to psychosocial interventions. 28. Assesses caregiver's ability to function adequately where applicable. 29. Assesses need for counseling related to risk assessment for pathological grief. 30. Assesses special needs related to cultural diversity including communication, space, role of family members and special traditions. 31. Identifies the developmental level of patient/family and obstacles to learning or ability to participate in the care of the patient. 32. Addresses patient/family questions and issues. 33. Identifies obstacles to compliance and assists in understanding goals of intervention. 34. Identifies support systems available to reduce stress and facilitate coping with end of life care. 35. Communicates psychosocial information to inpatient unit when level of care is changed. 36. Performs other duties as assigned by management. 37.
