We are accepting applications for a Social Services / Discharge Planner who can be available:

• Full Time

• Monday-Friday

• 8:30am-5:00pm

Compensation

• Starting wage between $20.00/hour - $24.00/hour depending on experience

Social Services / Discharge Planner

• Provides services to residents and families who are faced with social, emotional and medical stressors.

• Responsibilities include assessment of factors potentially affecting the resident, the family/caregiver system, and their environment, and educating about community resources and assisting in the referral process to ensure a safe discharge and/or appropriate continuity of care.

• Recognizes and plans for unique needs of physically disabled, cognitively disabled, chronically ill, and terminally ill residents.

• Empowers the resident and loved ones to participate in the environment to the fullest extent possible.

• Counsels for long-range planning and decision-making, crisis intervention, and strengthening support systems.

• Assists residents and their families with resources pertaining to discharge planning.

• Collaboration with the interdisciplinary team to meet residents expressed discharge goals.

Requirements

• Must have a Bachelor's Degree or Master's degree in Social Work or related human services field.

• Experience in healthcare preferred.

• Experience working with people with Alzheimer's disease and related memory disorders is preferred.
