Position Summary

The Social Services Director plans, organizes, develops, and directs the overall operation of the Social Services department to ensure all medically related emotional and social needs of patients are met in accordance with all applicable laws, regulations, and Life Care standards.

Education, Experience, and Licensure Requirements

  • Must be currently registered/licensed in applicable State. Must maintain an active license in good standing throughout employment.

  • Must have a bachelor's degree if working in a facility with 120 or more beds (see State law)

  • Two (2) years' experience in health care social work preferred

Specific Job Requirements

  • Must possess the ability to make independent decisions when circumstances warrant such action

  • Must be knowledgeable of practices and procedures as well as the laws, regulations, and guidelines governing functions in the post acute care facility

  • Must have the ability to implement and interpret the programs, goals, objectives, policies, and procedures of the department

  • Must perform proficiently in all competency areas including but not limited to: patient rights, and safety and sanitation

  • Maintains professional working relationships with all associates, vendors, etc.

  • Maintains confidentiality of all proprietary and/or confidential information

  • Must understand and follow company policies including harassment and compliance procedures

  • Displays integrity and professionalism by adhering to Life Care's Code of Conduct and completes mandatory Code of Conduct and other appropriate compliance training

Essential Functions

  • Must be able to implement a social services program that meets the medically related social and emotional needs of patients as well as State, Federal, corporate, and division guidelines

  • Must be able to act as patient advocate and provide education to staff regarding patient rights

  • Must be able to chart appropriately and timely

  • Must be able to assist patient and family through education, financial planning assistance, liaison with community agencies, etc.

  • Must be able to recruit, select, hire, train, evaluate, counsel, and supervise social services staff

  • Must exhibit excellent customer service and a positive attitude towards patients

  • Must be able to assist in the evacuation of patients

  • Must demonstrate dependable, regular attendance

  • Must be able to concentrate and use reasoning skills and good judgment

  • Must be able to communicate and function productively on an interdisciplinary team

  • Must be able to sit, stand, bend, lift, push, pull, stoop, walk, reach, and move intermittently during working hours

  • Must be able to read, write, speak, and understand the English language

The DTO is responsible for the overall operations of the therapy department, including high quality, cost effective delivery of patient care, managing therapy personnel, and developing and implementing approved department projects and goals. Integrates the hospital's mission and "Guiding Principles" into daily practice. Essential Functions: * Develops, maintains, and implements therapy policies and procedures that conform to current standards of therapy practice and operational policies while maintaining compliance with state and federal laws and regulations. * Directs the functions of the therapy department in accordance with departmental policies, procedures and standards. Oversees therapy department staffing, retention, and development and participates in coaching, discipline, and performance evaluations. Provides education, direction and mentorship of the therapy team's function, purpose, and goals. * Participates in developing the hospital's mission, strategic plans, budgets, resources allocations, operations plans, and procedures in conjunction with the hospital Administrator, other program directors, governing body, and medical staff to ensure clinical areas are represented in planning conducting organizational improvement. * Prepares annual capital and operating budget for therapy department. Monitors department expenses on designated schedule. Works with management staff of all third party payers on problems, solutions and new programs. * Collaborates with physicians, consultants, community agencies, and institutions to improve the quality of services and to resolve identified problems. * Collaborates on an interdepartmental level to integrate therapy services with the total patient's health care plan. Participates in all admission decisions, and monitors patient outcomes. * Provides an environment conducive to safety for patients, visitors, and staff. Assess the risks for safety and implements appropriate precautions. Qualifications/Skills: * Current license as Physical Therapist, Speech Language Pathologist, or Occupational Therapist required. * Current CPR certification required. * Three years' experience in a hospital therapy management preferred. Additional Qualifications/Skills: * Knowledge of and adherence to current therapy theory and practice. * Knowledge of accreditation standards to ensure adherence to all standards set forth by state and accrediting agencies of TJC and CMS. * Knowledge of clinical operations and procedures. * Demonstrates an understanding of treatment costs and financial support as they relate to quality and efficiency. * Demonstrates general computer skills including: data entry, word processing, email, and record management. * Effective organizational and time management skills. * Effective written and verbal communication skills. * Ability to maintain proper levels of confidentiality. * Ability to work closely and professionally with others. * Ability to maintain quality, safety, and/or infection control standards. * Ability to demonstrate leadership, clinical expertise, and appropriate interpersonal skills.
