Your Specific Responsibilities:

This position, an employee of the Suwannee County Health Department (SCHD), works to protect, promote, and improve the health of all people in Florida through integrated state, county, and community efforts. Finally, the employee is expected to perform with excellence by promoting quality outcomes through learning and continuous performance. This position reports to the Community Health Nursing Supervisor.

This is a position assigned to the Healthy Start Program that works under the supervision of the Community Health Nursing Supervisor to provide case management services to pregnant women and children, infant to age 3, enrolled in the Healthy Start Program in Suwannee and Lafayette counties. This position allows access to the client's records including immunizations, Healthy Start records, HMS, and Well Family System. All activities are provided within the requirements of the Healthy Start Standards and Guidelines.

System of care:

  • Provides Healthy Start screening, ongoing risk assessments and developmental screening to high risk pregnant women, children and their families.

  • Completes the Initial Assessment.

  • Establishes an Individualized Plan of Care for each client.

  • Contacts clients at clinic/medical appointments, by phone, letter, or home visits.

  • Services are coded and documented in the Well family System.

Care Coordination:

  • Provides direct care coordination services to enhance the participant's ability to access care, including follow-up of referrals, missed appointments and education (school attendance, GED classes).

  • Completes screenings as needed for economic, social, and physical needs.

  • Familiarizes self with community resources appropriate to the needs of the clients.

  • Completes developmental/social emotion infant assessments as indicated.

Provides education to Healthy Start clients and their families on:

  • Pregnancy

  • Infant growth and development

  • Breastfeeding, parenting, childbirth, interconceptional education, and tobacco cessation as training is obtained.

  • Car seat safety education classes after completing certification.

Assist Community Health Nursing Supervisor with record review, as needed and training of new Health Start staff. Attends training conferences, workshops, and any meetings related to the Healthy Start Program and/or CHD meetings.

Completes online travel. Maintains all trainings/certifications within required time frames. Other duties as assigned.

Required Knowledge, Skills, and Abilities:

Proficient in a word processing program i.e. Microsoft Word or similar program.

Personal vehicle may be needed for home visiting. Good interpersonal communication skills. Ability to work independently.

Preferred Qualifications:

Minimum of four-year college degree in one of the following areas: Social Sciences, a health-related field such as nursing, health education, health planning, or health care administration, social work or an Associate degree and licensure as a Registered Nurse with three years of public health/maternal-child health experience or licensure as a Licensed Practical Nurse with four years of public health/maternal child health experience or two years of college with three years of public health/maternal-child health experience.
