Examples of Duties Performed:
Responsible for planning, organizing, developing, and assisting with the overall operation of the Social Services Department for a long-term care nursing center in compliance with federal, state, and local regulations and center guidelines.
Consults with patients and family members concerning personal, emotional, medical, and social needs.
Assists patients in obtaining needed services when such services are not provided by the nursing center.
Provides Medicare, Medicaid, and other financial assistance information to patients and family members.
Responsible for meeting documentation requirements for long-term care guidelines related to social services.
To be considered for this opportunity applicants must have:
Bachelor's degree in social work, a related field, or nursing.
Two years supervisory experience in a business office and demonstrated knowledge of computers, office machines, and various business software applications.

Be capable of maintaining effective working relationships with patients, family members, staff and fellow department heads.
