Providence is calling a Social Services Clinician (MSW) (Case Management And Social Svc, Day Shift, 1.0 FTE) to Providence Alaska Medical Center in Anchorage, Alaska. In this position you will:

  • Evaluate patients and their related support systems with a complete psychosocial assessment.

  • Establish a therapeutic relationship and treatment goals with the patient or their surrogates.

  • Provide a provisional diagnosis based on DSM criteria that is inclusive of any mental, emotional, behavioral, addictive, developmental disorders and disabilities, psychosocial aspects of illness injury disability and impairment within a psychosocial framework.

  • Administer and interpret applicable measurements/tests (MSE, Edinburgh Depression Scale,) for the diagnosis.

  • Incorporate a plan or intervention that will address mental, social, emotional, behavioral developmental/age, addictive disorders, conditions and disabilities, and/or the psychosocial aspects of illness and injury as experienced by the patient and their family.

  • Demonstrate basic knowledge and develops expertise in the assessment and treatment of substance abuse, mental capacity and incapacity, mental illness, adult protection, child protection, domestic violence, sexual and physical assault, psychosocial crisis, bereavement, and mental illness.

Required qualifications for this position include:

  • Master's degree in social work (MSW) from an accredited school of social work

  • Knowledge of the biopsychosocial aspects of illness, disability and death and be able to apply this knowledge in a professional and understanding manner in direct social work services to patients and their families and in collaboration with all health care professional staff

  • Social Services Clinicians must have the ability to communicate well and interact with multiple health care professionals, including medical and allied staff

  • Proficiency in computerized documentation is required after orientation.

Preferred qualifications for this position include:

  • Alaska License in Clinical Social Work (LCSW) within 4 years of hire

  • 2 or more years of acute care Medical Social Work experience

  • Knowledge of the Alaska community resources

  • Experience with computerized documentation and medical records

Provide psychosocial assessments, diagnosis, and treatment, as well as consultation about patients and families to assist them and the health care team in coping with patient's hospitalization, illness, diagnosis, treatment, and/or life situation, including emotional, mental, and substance abuse disorders. In this job you will: * Assesses the functional and psychological needs of the patient within the framework of his/her developmental stage, functional abilities, cultural milieu, and support network. * Formulates a plan of intervention acceptable to the patient, family and medical team; proactively facilitates adjustment in the care plan or services when necessary to promote enhanced outcomes. * Documents all interventions in the patient medial record both timely and accurately including all elements of the discharge plan. * Provides advocacy, assistance, support, counseling and crisis intervention to patients and families. In the case of suspected abuse/neglect/exploitation, the Social Worker documents and reports where required. Assists with hospital-sponsored groups. * Functions in a liaison role between the hospital and community in making community resources available to the patient and family. * Collaborates with all members of the healthcare team to develop, manage and communicate discharge plans. Maintains current working knowledge of Medicare, Medicaid and other program benefits and criteria, particularly as they pertain to long-term and low-income patients. * Provides professional education to staff. Required for this position: * Licensed by the State of Alaska as a Social Worker. * Basic Life Support (BLS) certification is required, new hires have thirty (30) days from the date of hire to obtain BLS cards if not current at the time of hire. * Masters Degree in Social Work. * Requires a proficiency level typically achieved with 3 years acute care hospital experience. * Must have knowledge of government/community resources such as Medicare, Medicaid, long-term care or any other applicable resources/services. About St. Elias Specialty Hospital: St. Elias Specialty Hospital is owned by Providence Health System. The hospital has 59 beds, almost all private rooms. The facility features state of the art equipment to provide extended inpatient acute care for patients. At St. Elias Specialty Hospital, we provide customized, physician-driven services for patients requiring longer stays in an acute care environment due to multiple or complex medical conditions. We are not a permanent placement for patients; rather we are an answer for many patients who are acutely ill and need to take the first steps towards rehabilitation or home. Most of our patients are admitted from a traditional acute care hospital, often from the Intensive Care Unit.
