Position Summary

The Social Services Assistant provides assistance to the Social Services department to ensure all medically related emotional and social needs of patients are met in accordance with all applicable laws, regulations, and Life Care standards.

Education, Experience, and Licensure Requirements

  • Must be a high school graduate or equivalent

  • Prior health care experience preferred

Specific Job Requirements

  • Must possess the ability to make independent decisions when circumstances warrant such action

  • Must be knowledgeable of practices and procedures as well as the laws, regulations, and guidelines governing functions in the post acute care facility

  • Must have the ability to implement and interpret the programs, goals, objectives, policies, and procedures of the department

  • Must perform proficiently in all competency areas including but not limited to: patient rights, and safety and sanitation

  • Maintains professional working relationships with all associates, vendors, etc.

  • Maintains confidentiality of all proprietary and/or confidential information

  • Must understand and follow company policies including harassment and compliance procedures

  • Displays integrity and professionalism by adhering to Life Care's Code of Conduct and completes mandatory Code of Conduct and other appropriate compliance training

Essential Functions

  • Must be able to assist with the social services program to meet the medically related social and emotional needs of patients as well as Federal, State, corporate, and division guidelines

  • Must be able to document appropriately and timely, according to State law

  • Must be able to assist social worker(s) in supporting patients and families through education, financial planning assistance, liaison with community agencies, etc.

  • Must exhibit excellent customer service and a positive attitude towards patients

  • Must be able to assist in the evacuation of patients

  • Must demonstrate dependable, regular attendance

  • Must be able to concentrate and use reasoning skills and good judgment

  • Must be able to communicate and function productively on an interdisciplinary team

  • Must be able to sit, stand, bend, lift, push, pull, stoop, walk, reach, and move intermittently during working hours

  • Must be able to read, write, speak, and understand the English language

VIEW JOBS 2/18/2020 12:00:00 AM 2020-05-18T00:00 What You Will Do: The Patient Services Coordinator is directly responsible for scheduling the home visits for the field staff and assisting with the coordination of patient care. Works under the direction of the Clinical Manager to ensure the proper utilization of clinical resources to maximize care center capacity and utilization guidelines. Responsible for scheduling and communicating with field staff, patients and clinical manager regarding scheduling of patient services. * Ensures patients assessment visits including all Oasis visits are scheduled and performed timely. * Prepares weekly clinician schedules. Reviews clinician schedules daily or more frequently as needed to determine capacity opportunities and ensure productivity standards are being met. * Ensures clinicians are assigned and scheduled in most efficient geography, maximizing clinician efficiency, utilizing the lowest possible discipline, matching skill with required care, and optimizing clinician capacity. * Ensures missed visits are scheduled by the end of the current week or referred to the Clinical Manager for resolution. * Works collaboratively with field clinicians to ensure all patient visits are scheduled and completed as ordered and within the care center. * Manages calls from patients and field staff related to scheduling issues. Refers clinical and performance related issues to the Clinical Manager. * Monitors hospitalized patients, communicating and ensuring the team follows up as necessary. * Completes requested schedules ensuring all orders and services (e.g. lab draw, supervisory visits, add-on visits or disciplines etc.) are included as required. * Supports Clinical Manager and works collaboratively to ensure timely communication with patients, clinicians, referral sources, BD team and other office personnel. * Runs all applicable reports and responds to work flow taking appropriate actions. * Assists with internal or external transfer of patients between care centers and/or Hospice services. * If clinical and meet all the requirements of a field clinician, may provide direct patient care on an infrequent basis and only in times of emergency. * May be responsible for maintaining supply closet with routine supplies, ensuring supplies are within expiration dates and packaged appropriately, and serve as liaison with the field for patient supply needs. * In the absence of an intake coordinator, may be responsible for the Referral and Intake process within the care center. * Preforms other duties as assigned. Nurses will get priority for interviews. Required: * High School diploma or GED equivalent. * One (1 ) year of administrative experience in a healthcare environment Preferred: * One (1 ) year of scheduling experience in a healthcare environment.
