Position Summary

The Social Services Assistant provides assistance to the Social Services department to ensure all medically related emotional and social needs of patients are met in accordance with all applicable laws, regulations, and Life Care standards.

Education, Experience, and Licensure Requirements

  • Must be a high school graduate or equivalent

  • Prior health care experience preferred

Specific Job Requirements

  • Must possess the ability to make independent decisions when circumstances warrant such action

  • Must be knowledgeable of practices and procedures as well as the laws, regulations, and guidelines governing functions in the post acute care facility

  • Must have the ability to implement and interpret the programs, goals, objectives, policies, and procedures of the department

  • Must perform proficiently in all competency areas including but not limited to: patient rights, and safety and sanitation

  • Maintains professional working relationships with all associates, vendors, etc.

  • Maintains confidentiality of all proprietary and/or confidential information

  • Must understand and follow company policies including harassment and compliance procedures

  • Displays integrity and professionalism by adhering to Life Care's Code of Conduct and completes mandatory Code of Conduct and other appropriate compliance training

Essential Functions

  • Must be able to assist with the social services program to meet the medically related social and emotional needs of patients as well as Federal, State, corporate, and division guidelines

  • Must be able to document appropriately and timely, according to State law

  • Must be able to assist social worker(s) in supporting patients and families through education, financial planning assistance, liaison with community agencies, etc.

  • Must exhibit excellent customer service and a positive attitude towards patients

  • Must be able to assist in the evacuation of patients

  • Must demonstrate dependable, regular attendance

  • Must be able to concentrate and use reasoning skills and good judgment

  • Must be able to communicate and function productively on an interdisciplinary team

  • Must be able to sit, stand, bend, lift, push, pull, stoop, walk, reach, and move intermittently during working hours

  • Must be able to read, write, speak, and understand the English language

About The Position The Adaptive Services Recreation Leader II leads, plans, and conducts recreation activities at a recreation site, facility, or specialty area in Community Services for people with disabilities. Examples are facilitation of positive social interaction between a child with a disability and his/her non-disabled peers in an after school program; coaching adaptive sports; and, modifying activities to meet participants individual abilities in an inclusive setting. Please note: Due to this position being part-time, the classification is unclassified, meaning it is at-will. Selection Process Applicants whose education, training and experience most closely meet the needs of the position may be invited to participate in a selection process to include a panel interview and an assessment exercise. Successful candidates will receive a post-offer, pre-employment background screening to include: * Fingerprinting * Criminal Background screening for the past 7 years * Motor Vehicle Department Records Check * Drug Screening Minimum Qualifications Education and Experience * A minimum of 6 months work experience in a recreation related field and/or 6 months of paid customer service experience. College coursework in recreation or related field from an accredited institution may be substituted for some recreation experience. Licensing, Certifications, and Other Requirements * A valid driver license with no major driving citations within the past 39 months may be required. Other pertinent licenses and/or certifications may be required of some positions depending on division/department/section assignment. Essential Functions Performs duties and responsibilities commensurate with assigned functional area which may include, but are not limited to, any combination of the following tasks: * Plan, implement, and evaluate recreation programs for youth, teens, and/or adults with developmental, physical, emotional and/or learning disabilities. * Provide physical assistance to participants as needed - excluding personal care services. * Ensure that activities are modified (when possible) to meet individual abilities and accommodate limitations of participants. Ensure that activities are age and ability appropriate. Ex: Providing step by step instructions to individual participants during play when participating in group games or providing brightly colored balls, paddles, goals, etc. during sports games if participant has visual difficulties are examples of reasonable accommodations. * Communicate regularly (at least twice a week) with parents/guardians of ARS participants to keep them apprised of upcoming activities, daily participation, positive accomplishments, and any behavior related incidents. * Assist with thorough cleaning of activity area at the end of each day (may include mopping, sweeping, cleaning glass windows & doors, emptying garbage, washing dishes, organizing storage areas, etc.) Work Environment/Physical Demands The physical demands and work environment characteristics described here are representative of those that must be met by an employee to successfully perform the essential functions of this position or that an employee encounters while performing the essential functions of this position.
