Social Services Assistant

Examples of Duties Performed:
Assists with the planning, coordination, and development of the Social Services Program to provide for the psychosocial needs of patients and families served by the center.
Participates in family and patient activities designed to promote social interaction, reality orientation, and intellectual stimulation.
Counsels patients and families including dealing with feelings about death and dying and other emotional, mental, environmental or physical limitations.
Completes the social service portion of the MDS.
Understands appropriate documentation for Advance Directives (Living Will, DNR, DPAH).
Coordinates admissions and discharges.
Assists with the coordination of family and community resources as needed to solve financial needs and to promote emotional security.
To be considered for this opportunity applicants must:
Should be a high school graduate or its equivalent.
Have a minimum of two (2) years related work experience in a hospital, skilled nursing center, or similar medical setting.
Possess experience in a business office and demonstrated knowledge of computers, office machines, and various business software applications.

Possess ability to communicate effectively, both orally and in writing.
Be capable of maintaining effective working relationships with patients, family members, staff and fellow department heads.
