Social Services Assistant

Monday - Friday

Examples of Duties Performed:

  • Assists with the planning, coordination, and development of the Social Services Program to provide for the psychosocial needs of patients and families served by the center.

  • Participates in family and patient activities designed to promote social interaction, reality orientation, and intellectual stimulation.

  • Counsels patients and families including dealing with feelings about death and dying and other emotional, mental, environmental or physical limitations.

  • Completes the social service portion of the MDS.

  • Understands appropriate documentation for Advance Directives (Living Will, DNR, DPAH).

  • Coordinates admissions and discharges.

  • Assists with the coordination of family and community resources as needed to solve financial needs and to promote emotional security.

Requirements:

To be considered for this opportunity applicants must:

  • Should be a high school graduate or its equivalent.

  • Have a minimum of two (2) years related work experience in a hospital, skilled nursing center, or similar medical setting.

  • Possess experience in a business office and demonstrated knowledge of computers, office machines, and various business software applications.

  • Embrace the standards, values, and beliefs of the organization.

  • Possess ability to communicate effectively, both orally and in writing.

  • Be capable of maintaining effective working relationships with patients, family members, staff and fellow department heads.
