Part time Social services assistant
As the Social Services Assistant you will Assist the resident/family in achieving smooth nursing home transition at admission through discharge. Support the psychological needs of the resident.
SPECIFIC DUTIES AND RESPONSIBILITIES:
Assure the psychosocial needs of the resident are assessed and needs met
Provide families/residents with information on Residents rights and Advance Directives
Serve on committees and conduct/participate in meetings as they relate to resident care (care plans, Family Council, Alzheimer's Support Group, etc.)
Serve as resident/family liaison regarding discharge planning
Act as back up to Admissions Director regarding admission inquiries, tours, etc.
Brings any concerns, issues, needs or changes to the attention of the Administrator
Attend required facility meetings and MOD rotation schedule
Maintain confidentiality of information
Maintain a written record of the residents' complaints and/or grievances that indicates the action taken to resolve the complaint and the current status of the complaint
Maintain the department in regulatory compliance
Participate in facility surveys made by authorized government agencies
Perform administrative requirements, such as completing necessary forms, reports, etc., and submitting such to the Director as required
All other related duties ad assigned by Social Services Director
Must be a Licensed Social Worker in the State of Delaware
1 year Social Work Experience preferred
Healthcare Industry preferred
