The individual in this role serves as a member of the Developmental Disabilities Mobile Crisis Response team. The focus of this program is to provide time-limited home and community based crisis services that support individuals with developmental disabilities in the community.
An essential component of this program is the assessment of the individual situation to determine the appropriate response to the crisis. When the Developmental Disability Mobile Crisis team is dispatched to the crisis location, the team assesses the need for a referral or crisis services. Crisis services occur through intensive on-site or off-site supports. The crisis supports are provided on a time-limited basis.
The Social Service Technician I works as a member of a Mobile Crisis team providing services to persons with Developmental Disabilities (DD). Responsibilities include but are not limited to the following:
Interviews clients and/or families using acceptable techniques to obtain necessary information regarding medical, social, and/or emotional background
Contacts other healthcare professionals within or outside the organization as necessary to obtain background information on clients
Attends and participates in treatment team meetings when requested
Provides appropriate input to the treatment team regarding consumer's physical/psychosocial status based on interviews and observation of client
Documents concise, accurate, and clear observation notes utilizing the Electronic Health Record (EHR)system
Makes referrals to appropriate agencies for client support services
Notifies appropriate professional(s) of any unusual client incident/situation in a timely manner
Prepares accurate periodic and adhoc reports as required
Provides appropriate emotional support and encouragement to clients and families and assists in the resolution of minor problems
Provides appropriate information and/or input in meetings pertaining to case management and/or case collaboration
High school diploma or GED and one year experience in a social services related position Or Completion of two years of college with at least 15 credit hours in a social services related field
High school diploma or GED AND One year of experience in a social services related position OR Two years at an accredited college or university with at least 15 credit hours in a social services related field.
