Educational Requirements

Centers with more than 120 Beds: Bachelors in Social Work or a Bachelor's in Human Services field, including but not limited to Sociology, Special Education, Rehabilitation Counseling and Psychology; Licensed where required by law.

Centers with 120 Beds or less: High School Diploma required; Bachelors in Social Work or a Human Services field desirable; Licensed where required by law.

Position Requirements

For Centers with more than 120 Beds: One year of supervised Social Work experience in a health care setting working directly with geriatric individuals.

For Centers with 120 Beds or less: Two years of supervised Social Work experience in a health care setting working directly with geriatric individuals desirable.

Job Specific Details:

Responsible to provide medically related social work services so that each patient may attain or maintain the highest practicable level of physical, mental and psychosocial well-being.

Responsibilities include but not limited to: identifies cognitive impairments, communicates effectively with the interdisciplinary team, maintains accurate and timely documentation, supports each patient's right to self-determination, facilitates communication among patients providing risk/benefit information regarding significant health care decisions, coordinates care conferences, coordinates the discharge planning process, makes appropriate referrals, liaison with psych consultants, provides assistance with patient's admission, informs of medicare/medicaid program benefits, encourages patient autonomy, provides orientation and in-service training.

Education: High school diploma required; bachelor's degree in social work or a human services field desirable

Experience: Two years of supervised social work experience in a health care setting working directly with geriatric individuals desirable.
