Summary:

Under the supervision of Extended Care Services Director, the Social Adjustment Services Case Worker's primary responsibility is to ensure that services are provided in a planned, effective and timely manner to eligible clients. The Social Adjustment Services Case Worker will ensure that the services are appropriate to meet the needs of the clients and will contribute to self-sufficiency of participating clients. The Social Adjustment Services Case Worker is responsible for timely and accurate case documentation and maintaining case files. The duties extend to other refugee support programs that provide a holistic approach towards assisting refugees.

Position Responsibilities

  • *Works with client's other refugee programs to develop tailored service plans that address individual needs, including securing safe and suitable housing, obtaining essential items such as clothing and food, arranging transportation, and accessing employment, education, medical and mental health care, and legal services.

  • *Provides direct service support and referrals as needed in collaboration with other refugee programs in areas of housing, employment, job training, education, legal services, counseling, health and emergency in alignment with the clients' service plan.

  • *Provides personalized case management services aimed at guiding clients toward achieving economic independence and social integration.

  • *Regularly reports on client interactions, service visits, and any significant issues or concerns to the Director/Manager and other staff during required staff meetings or as necessary.

  • *Manages and updates client information and tracks program objectives efficiently, utilizing both an external online refugee database and internal data management spreadsheets.

  • Reviews and maintains accurate and up-to-date client files, including intake, assessments, and filing.

  • Engage in ongoing professional development through training sessions and webinars to enhance skills and knowledge, ensuring the delivery of high-quality service to clients.

  • Demonstrate cultural sensitivity and awareness of the socioeconomic backgrounds of the clients served tailoring approaches to meet their diverse needs effectively.

  • Commit to daily adherence to safety protocols and training, proactively implementing measures to safeguard personal well-being and that of others.

  • Uphold the utmost confidentiality regarding all aspects of work within the organization. This includes protecting the identities and information of clients, the details of services provided, the identity and contributions of donors, any internal or external investigations, and their outcomes, as well as sensitive financial data.

  • Other duties as assigned by the Program Director, Manager and Coordinator.

Requirements

Minimum Qualifications:

  • Education

o bachelor's degree preferred but not required; or equivalent of 2 - 4 years' experience in a social service field.

  • License and Credentials

o Reliable transportation

o Valid driver license

o Valid vehicle insurance

o Ability to operate an agency 12 passenger van on an as needed basis for program activities and events.

o Must be 21 years of age and adhere to the CCAOSA policy, which will not allow anyone under the age of 21 to operate an agency vehicle

o Must have a cleaning driving record

Minimum Knowledge and Skills:

  • Proficient in managing client documentation and maintaining accurate records.

  • Competent with personal computers, including proficiency in various software applications, email, and internet usage.

  • Must be detail oriented, organized, self-motivated, and capable of working both independently and collaboratively.

  • Excellent written and verbal communication abilities.

  • Strong critical thinking and problem-solving skills.

  • Ability to drive and maintain reliable personal transportation.

  • Capable of safely operating a 12-passenger vehicle as required.
