We want to see you excel, because we believe that your success is our success.We currently have an opening for an SNF Placement Case Manager:The SNF Placement Case Manager performs triage functions by providing support and guidance, to After Hours team by assisting with transferring patients from Acute setting to SNF, Sub-acute, Congregate Livings and/or Recuperative Care Centers. Afterhours SPT Case Manager responds to and coordinate service request for transfers, request for SNF/Sub-acute/Recuperative Care admissions and facilitate discharges to lower levels of care to prevent additional bed days in Acute and delay of transfers. Demonstrates self-direction in establishing priorities and working with routine supervision. Working collaboratively with After Hours SPT Coordinator and After Hours Team after business hours and weekends. Maintains high level of precision in judgment, accuracy of assessment, and skill in problem solving under stress. Relates and communicates well over the telephone to all callers.*
  • Facilitates transition of members from Acute setting to Skilled Nursing Facilities, Sub-Acute Facilities, Congregate Livings and Recuperative Care Centers. * Communicate with After Hours Inpatient team * Clinical review of SNF placement requests and clinical progress notes * Receives, triages, and responds to telephone calls from facilities requiring medical and nursing inquiries * Performs comprehensive review of clinical records, history & physical, evaluation of Skilled need and advice while meeting production standards. * Assesses patient problems utilizing approved MCG guidelines, directing access to care to include Skilled Nursing Facilities, Sub-acute Facilities, Congregate Livings and Recuperative Care Centers * Follows established clinical protocols/guidelines * Directs patient to appropriate levels of care based on assessment within the established guidelines. * Communicates to After Hours Management and escalate issues as necessary * Facilitates authorizations for vendors and/or outside services as appropriate. * Obtain LOA for non-contracted facilities as needed * Documents all calls according to established standards in REA/LEA* Documents all SNF inquiries and member disposition in the SNF Placement Team log daily * Participates in quality monitoring process by daily verbal endorsement between SNF Placement Team business hours and After Hours Team * Translates oral information into concise and accurate written documentation using standard medical guidelines and abbreviations. * Shift summary to be sent to the Director, Manager and Lead of SNF Department * Recognizes financial, medical and legal risks based on data collected during customer interactions and follows appropriate procedures. * Demonstrates customer service-oriented behavior at all times. * Acts as a mentor as requested by supervisor. * Expedites emergency calls as needed. * Participates in regular staff meetings as scheduled. * Uses, protects, and discloses patients' protected health information (PHI) only in accordance with Health Insurance Portability and Accountability Act (HIPAA) standards. * Consistently exhibits behavior and communication skills that demonstrate commitment to superior customer service, including quality, care and concern with each and every internal and external customer. * Performs additional duties as assigned. * Adheres to Code of ConductSkills * Good judgment and critical-thinking skill * Compassionate and caring manner with patients * Telephone "charisma"* Forms instant rapport * Customer serviceoriented * Helpful, warm, open, and inviting * Excellent communication skills (written, verbal, and technical)* Articulate * Able to communicate well at fifth- to eighth-grade level * Bilingual language skills a plus * Concise, clear documentation skills * Excellent listening skills * Excellent negotiation skills * High levels of experience or expertise in: * Crisis intervention * Teaching/coaching * Disease management * Discharge Planning * Attributes*

Essential Duties and Responsibilities include the following: 1. Primary focus will be developing individualize care plan and documentation to meet all regulatory requirements, NCQA, CMS, State (APL/DPL) on turned around time. 2. Review and analyze available clinical data, develop a person centered evidence based care plan by identifying member problems, goals, barriers, developing care plans base on SMART goals. 3. Keeps Core Case Manager, coordinator and leadership informed of any clinical findings that requires urgent intervention or immediate outreach, urgency of the clinical requests when necessary. 4. Demonstrates the ability to follow through with requests, collaboration, sharing of critical information, and getting back to individuals in a timely manner. 5. Functions as liaison between administration, members, physicians and other healthcare providers. 6. Interacts professionally with member/family/physicians and involves member/family/physicians in formation of the plan of care. 7. Develops an outcome-based plan of care, based on the member's HRA, medical record, claims, authorization history and other relevant clinical findings. 8. Documents member care plans, and other pertinent information completed in the member's medical record in accordance with the FOCUS Charting methodology, nursing standards, and company policies and procedures. 9. Identify and refer the member if they will benefit the community visits (hospital, home visits) as needed to assess patient progress and meet with appropriate members of the patient care team. 10. Review utilizations associated with IP/ED, Skilled Nursing facility Services and develop a comprehensive care plan identified for the utilization of service. 11. Communicates appropriately and clearly with physicians, in patient case managers and Prior-Authorization nurses 12. Identifies and addresses psychosocial needs of the members and family and facilitates consultations with Social Worker, as necessary. 13. Identifies and addresses pharmacological needs of the members and facilitates consultations with the pharmacy department for medication review and medication reconciliation every initial Care Plan, annual, bi-annual and every transition of care. 14. Identifies community resources to address needs not covered by the member's benefit plan, and coordinates member benefits as needed, with the health plan. 15. Participates in the efficient, effective and responsible use of resources such as medical supplies and equipment. 16. Ability to collaborate and communicate with all members of the healthcare team (concurrent review, pre-authorization, PCP/SPC, Social Services, and Pharmacy) to coordinate the continuum of care of developing plans for management of each case. 17. Responsible for the identifying members that are appropriate for hospice conversion or Palliative care. 18. Other duties as assigned. Education and/or Experience: 1. Graduate from an accredited Registered Nursing Program or Licensed Vocational Nursing Program. 2. Current CA RN, or LVN license, current CPR certification, valid CA Driver's license. 3. 3 years acute care or case management experience preferred. 4. 2-3 years of utilization or HMO experience preferred. 5. Typing 40 words per minutes with accuracy. 6. Must have valid CA DL and provide proof of vehicle insurance. 7. Knowledge of computers, faxes, printers and all other office equipment. 8. Knowledgeable in MS Office Programs (i.e., Word, Excel, Outlook, Access and PowerPoint) 9. Optional on call duties as assigned.
