What you'll do:

  • Install and service authorized smart home technology in customer homes

  • Ensure on-time arrival to daily customer appointments

What we're looking for:

  • Experience as a construction worker, electrician or other technical is helpful

VIEW JOBS 10/4/2019 12:00:00 AM 2020-01-02T00:00 Christian Care Communities and Services is seeking a Home Health Admissions and Nursing Supervisor to directly supervise home health direct care teammates, perform direct care according to current standards of practice, state and federal regulations and facility policy while overseeing the health and wellness of the program's patients/clients. The primary responsibilities of this role include: * Completes initial comprehensive and ongoing assessments to identify the physical, psychosocial, and environmental needs of home health patients/clients. Completes assessments at appropriate time points, including Outcome and Assessment Information Set (OASIS) or other assessments as appropriate to the patient/client. * Regularly assesses and re-evaluates the patient/s/client/s nursing needs and evaluates the outcome of care. * May perform staff competency assessment including aide competency evaluations. * Performs those services requiring nursing skills that are ordered by the physician and indicated in the plan of care. Counsels the patient/client, caregiver, and family in meeting nursing and related needs. * Develops, initiates and evaluates the plan of care in partnership with the patient/client, representative (if any), and caregiver(s) as necessary to ensure quality and continuity of care. Initiates appropriate preventive and rehabilitative nursing procedures. Uses nursing skills to assess effectiveness of plan and make revisions as indicated by skilled assessment. Refers to other services as needed. * Provides patient/client and caregiver education and counseling. * Plans for the discharge of the patient/client from services. * Communicates with all physicians involved in the plan of care and other health care practitioners (as appropriate) related to the current plan of care. Integrates orders from all relevant physicians involved into the plan of care and ensures the orders are approved by the responsible physician. * Supervises, teaches, and provides clinical direction and leadership to clinical nursing and aide personnel. Assigns home health aides to specific patients/clients. Supervises LPNs/LVNs and paraprofessionals providing services to patients/clients. Qualified candidates will be a graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing, as well as possess an active Registered Nurse license to practice in state of Texas or possess a multi-state license issued by a state member of the National Nurse Licensure (NLC), A minimum of two years of nursing experience in a clinical care setting and one year of home health experience required.
