Position Summary:

The Skin Care Program Coordinator assists the Skin Care Program Manager of Northeast Hospitals, providing clinical consultation for patients with wounds, pre- and post-op ostomy care/management, and/or patients at risk for skin breakdown. The Skin Care Program Coordinator follows established standards of care and assists in the development of evidence based practice.

The Coordinator will have the clinical competencies to provide specialize consults to meet wound, ostomy, continent patient needs, and provide training programs to ensure clinical expertise within the scope of WOC nurse guidelines. The Skin Care Program Coordinator Co-leads the Skin Care Team to develop and implement strategies to prevent and treat hospital-acquired pressure ulcers. In collaboration with the medical staff the Skin Care Program Coordinator provides consultation for complex wound, ostomy and incontinence patients. Must have certification from recognized WOCN Program

Qualifications

Education/Experience:

Must have certification from recognized WOCN Program. Must be a Registered Nurse (RN) licensed in Massachusetts.

Bachelor degree required, Masters preferred. Minimum of 5 years of licensed professional nursing experience required. Minimum of 3 years of experience in wound /Ostomy care management and education.

About the Job POSITION SUMMARY: The Behavioral Health Community Partner (BHCP) program at Lahey Health Behavioral Services is a new, innovative Care Management program for MassHealth members that are part of the new Accountable Care Organization (ACO) and or a Managed Care Organization (MCO). Lead care coordinators are senior community health workers (CHWs), health outreach workers, peer specialists, recovery coaches The Lead Care Coordinator provides supervision to care coordinators and also performs outreach and engagement, care coordination, care management, and care transition functions and activities for individuals with high behavioral health needs and enrollees in the BH CP program. The Care Coordinator, under the direction of the BH CP Clinical Care Manager, is a Member of the Care Team and a key contributor to the Care Planning process. The Lead CC monitors the Members adherence and response to the Care Plan. QUALIFICATIONS: * High school graduate or equivalent required. Bachelor's Degree from an accredited university in psychology, social work or related human services field preferred * 1-3 years' experience in community-based behavioral health support program preferred. * Ability and desire work with a diverse client population * Strong communication skills (both written and verbal) * Strong time management and organization skills * Must demonstrate good boundaries regarding confidentiality and personal relationships * Strong ability to evaluate what is needed by each individual and adjust approach accordingly. * Experience with accessing local resources and navigating mental health and/or substance abuse treatment systems. * Proficiency with electronic health record documentation or ability to complete documentation electronically is required.
