Four County is a growing non-profit Community Mental Health Center currently serving the communities of North Central Indiana since 1975. Position Responsibilities include but are not limited to:

  • Responsible for case management, care coordination, and life skills training for assigned caseload.

  • Delivery of assessment, treatment planning, intervention, and monitoring of goal achievement for consumers.

  • Transportation of consumers as relevant to their care plan needs

  • Achievement of service delivery goals and timely, accurate documentation/reports.

  • Work is very broad in scope and requires a high level of professionalism.

  • Desire to work in home-based and community settings.

Base pay is dependent on education, experience, and specialty:

  • Bachelor's Degree: $15.50 - $20.91
  • Associate's Degree (or less): $14.50 - $19.91
  • Bilingual premium available for individuals fluent in Spanish or Burmese

Additional Compensation:

  • Eligible for up to $3,550/year in productivity and quality bonuses

  • Eligible for a retention bonus of $250 every 3 months

  • Eligible for $150/month in student loan repayment

  • Earn $750 for every person you refer and is hired and receive an additional $500 every 6 months as long as you both remain employed.

Qualifications

Requirements:

  • High School graduate or GED with two years work experience in social services or currently working toward Associate's or Bachelor's degree.

  • Associate or Bachelor's degree in social services, social work, psychology or related behavioral health field preferred but not required.

  • If working in Substance Abuse case management must have or be working towards either state or national certification in addictions, licensure, or certification as recovery coach.

  • Must maintain reliable transportation

  • Must have a valid Indiana Driver's License with meet Center policy requirements for driver's license points.

  • Must maintain vehicle insurance requirements per Center policy.

Description We are seeking a Food Service Manager to support our Patient Services Operation. The Food Service Manager - Patient Services will be responsible for developing and executing dining solutions to meet paitent and customer needs and tastes. This person will utilize technical training and interpret government mandated nutritional standards in the healthcare industry on food health to develop healthy menu implementation plans. Essential Functions: * Leadership: Leverage Aramark's coaching model to engage and develop team members to their fullest potential. Reward and recognize employees. Ensure safety and sanitation standards in all operations. * Client Relationship: Maintain effective client and customer rapport for mutually beneficial business relationships.
