Skilled Nursing Rn/Lpn Unit Manager - FT Days - Fauquier Health And Rehab

The Nurse Care Manager works directly with the primary charge nurse. This position provides nursing care to the adult and geriatric population and assists the unit secretary with maintaining documentation compliance for the assigned unit.

• Serves as first point of contact for family members, residents, and staff on assigned units for issues, concerns, questions.

• Evaluate nursing care provided by continual review of the effectiveness of the nursing care plan. Collaborates with nurses and CNA's to make appropriate revisions to achieve the desired outcomes.

Minimum Education

Licensed Practical Nurse certificate or degree required. Registered Nurse Associate's degree preferred.
