What We Are
We are a Federally Qualified Health Center (FQHC) serving all those in need in the Sacramento area. We have been a level 3 Patient Centered Medical Home since 2011 and were the first to earn that designation in Sacramento. Integrated, comprehensive and supportive services to maximize health outcomes. The founding values of One Community Health were based on providing a safe and welcoming place for people who often experienced social exclusion and discrimination.
We offer a choice between two carriers: Blue Shield and Kaiser Permanente.
One Community Health pays about 90% of the Employee only premium and pays 75% of the costs for dependents or partners/spouses.

About the Position
This position is responsible for providing primary care to One Community Health patients while also supporting the Chief Medical Officer.
Perform as an active member of the medical team by providing patient care.
Provide site-based oversight of medical services delivered by providers in the practice, which will include Physicians (MD/DO's), Nurse Practitioners, and Physician Assistants.
Ensure that patient care/staff practice meet both safety/risk management standards and legal/regulatory requirements.
Works collaboratively with the Health Center Director and other clinic managers and supervisors to ensure that clinic operations and care we provide is safe, effective and efficient, meets the needs of the population served, supports the professional development of staff and is continuously improving.
Participates in shaping overall policies for One Community Health and helps to maintain compliance with all applicable laws and regulations.
Fully licensed to practice in the state of California
Processes a DEA Certificate
2 years Supervisory/Management experience
Previous experience in clinic and/or with HIV+/AIDS patients desired.
