IDS International is looking for a Transition Counselor to support the Soldiers for Life

  • Transition Assistance Program (SFL TAP). This role may require travel in order to accomplish mission goals to include evenings and weekends with the ability to use all modes of transportation; along with flexibility to work non-standard hours as needed in order to properly serve and assist clients. This role is located at the Redstone Arsenal in Huntsville, AL and is contingent upon award.


  • Provide individual counseling for personal interaction enabling users to ask questions, seek clarification and obtain assistance resulting in an ITP.

  • Schedule, set-up classroom, announce and plan local briefings and coordinate DOL Employment Workshops and VA Benefits Briefings as required to meet the needs of the Soldiers.

  • Provide Transition Briefing which provides clients with required information regarding available transition services and eligibility requirements for transition benefits.

  • Notify the eligible Soldiers of the required pre- separation counseling; conduct individual counseling; develop an ITP; develop a rsum of choice (federal or private sector); discuss and develop individual transition goals; learn about continuum of service.

  • Provide follow-on counseling and assist in completing the deliverable for the Soldier's gap analysis; identify, schedule, and conduct SFL TAP employment assistance workshops; as well as conduct regional outreach, pre- separation counseling and assistance.

Required Qualifications:

  • Master's degree preferred in Training and Development, HR Management, Counseling, or related field and minimum 2 years career counseling, education, or related experience. Bachelor's Degree accepted with additional experience.

  • Minimum of two years of experience in Military lifestyle through personal and/or professional experience with Military personnel and Military Family members highly desired, along with in-depth understanding about the Military culture and the Command structure.

  • Demonstrated experience in conducting program evaluations and use of customer feedback to improve overall program effectiveness/impact highly desired.

  • Candidates must be a U.S. Citizen and successfully undergo a background investigation (National Agency Check).

  • Individual must be familiar with standard concepts, practices, and procedures within the field.

Computer Skills:

  • Working knowledge of Microsoft Office (Word, Excel, and PowerPoint).

Must be an effective communicator both verbally and in writing. Most positions require ability to work on a desktop or laptop computer for extended periods of time. Computer activities include reading, reviewing and analyzing information, as well as providing recommendations, summaries, and reports in written or verbal format. Additionally, positions require ability to sit at a workstation for extended periods; use hand(s) to handle or feel objects, tools, or controls; reach with hands and arms; as well as speak and hear.

Employee may be required to occasionally lift and/or move moderate amounts of weight, typically less than 20 pounds. Regular and predictable attendance is essential.

VIEW JOBS 3/25/2021 12:00:00 AM 2021-06-23T00:00 What You Will Do The Care Transitions Coordinator (Operations) is a clinical liaison position between health care providers to ensure continuity of care for patients transitioning from a facility to home care or hospice environment. The position has two separate and distinct general responsibilities: (1) following the receipt of a valid referral for home health or hospice services, directly communicating with and assessing the patient to improve the patient's transition from the inpatient to the home setting; and (2) maintaining the referral relationships of the agency within the facility and referral source, in accordance with Amedisys policies and procedures. Required: * 1 years previous experience assisting patients through the continuum of care and the transition from hospital to home care, required. * Current CPR certification * Maintain facility qualifications for outside vendors, ie: annual TB testing and Hep B vaccination * Competent organizational skills; * Ability to handle stressful situations/deadlines; * Excellent oral/written communication and interpersonal skills; * Must demonstrate the ability to communicate effectively with all members of the health care team; * Demonstrates desire to work in a clinical role, while maintaining a relationship with referral sources to ensure quality transitions of referrals * Ability to forecasts needs and set priorities. * Proficient in computer-based skills. Must have working knowledge and practical application experience with general office computer systems (i.e. Microsoft Excel), internet, email, desktop navigation. Preferred: * RN strongly preferred.
