Overview
The Service Coordinator service is to establish, along with the member, a life-long, person-centered, goal-oriented process for coordinating each person unique preference and requirements. The Service Coordinator will assist individuals with developmental disabilities to live fully in the community. This includes living where any citizen in the community may live, developing relationships with people who do not have disabilities. The Service Coordinator will assist individuals with developmental disabilities by providing support in practical and familiar routines such as shopping, cooking, personal care, housekeeping, banking, socializing and recreation. The supports (both natural and paid), range of services, instruction and assistance needed by persons with intellectual and/or developmental disabilities. It is designed to ensure accessibility, accountability and continuity of support and services. This service also ensures that the maximum potential and productivity of a member is utilized in making meaningful choices regarding their life and their inclusion in the community.
Responsibilities
Assists the member and/or legal representative with re-determination of financial eligibility as required
Provides oral and written information about the Waiver provider agency's rights and grievance procedures for members served by the agency
Assists with procurement of all services that are appropriate and necessary for each member within and beyond the scope of the Waiver Program including annual medical and other evaluations as applicable to the member
Acts as an advocate for the member by coordinating opportunities for connecting with community resources
Promotes a valuable and meaningful social role for the member in the community while recognizing the member's unique cultural and personal value system
Provides ongoing supports to maintain desired lifestyle
Interfaces with the client/guardian and Waiver Support Coordinator quarterly as outlined in the iBudget handbook
Maintains linkage, negotiation of services, submission of information, coordination of choice of appropriate assessment
Assists the third-party benefits, including but not limited to Social Security, Food Stamp applications, recertification for Housing/Section 8 services
Within 30 days of receiving the Support Plan, create the transition/implementation Plan, Initial Housing Survey, Functioning Community Assessment Forms, annual Summary, Financial Profile no more than 10 days following housing selection
Submits request for additional funding to Waiver Support Coordinator prior to signing of lease/mortgage documents, and other required documents as outlined in the iBudget handbook
Supports the member as necessary to convene and conduct Quarterly meetings
Accesses the necessary resources and make referrals to qualified service providers and resources and monitor that service providers implement the instructional, behavioral and service objectives
Schedules and transports consumers to their schedule physician appointments and serves as an advocate assisting with informing the physician with critical information that is critical to the ongoing care of the individuals
Ensures that all bills related to the individual is paid on time monthly and as scheduled
Assists with locating housing and needed household supports
Teaches the acquisition, retention or improvement of activities of daily living
On call 24 hours a day 7 days a week
Other duties as assigned
Qualifications
Bachelor's Degree with a major in Education, Rehabilitative Service, Business or related degree
Associate Degree and have two years of documented direct experience with recipients with developmental disabilities
One year of college and three years of documented direct experience in working with recipients with developmental disabilities
Four years of direct professional experience in working with recipients with developmental disabilities nay substitute for college on a year for year basis the provider must hold a high school diploma or GED diploma
Florida Waiver experience and Medicaid service delivery knowledge are beneficial
Ability to communicate verbally and in writing
Ability to travel between service sites
Working knowledge of Federal, State and local regulations concerning services to individuals with intellectual and/or developmental disabilities
Good organizational skills and attention to detail and be capable of working flexible hours
Microsoft Word and Excel skills preferred
Successfully complete the APD course entitled "Supported Living Pre-Service", prior to serving any clients
Successfully complete APD's course entitled "Introduction to Social Security Work Incentives" within one year of receiving their certificate of enrollment as a Supported Living Coach
Complete 8 hours of annual in-service training
