Service Coordinator - Family Netwroks Services

Our teamof dedicated professionals works closely with each child and their families to address their emotional and behavioral challenges. We actively involve parents, family members, caregivers, schools, and other supports in a holistic, strengths based, trauma informedtherapeutic approach.
The Family Networks Service is currently seeking a Service Coordinator.
1.Assess and review DCF referrals to be processed appropriately.
2.Utilize designated Level of Service Tool to evaluate appropriate referrals to step-down clients from residential to home placement.
3.Oversee emergency placement to shelter/specialized care (respite pre-residential).
4.Oversee step-down of youth from residential to home placement.
5.Organize and lead Family Networks diversion teams.
6.Make appropriate network and community service referrals.
7.Coordinate services with specialized service managers.
8.Facilitate Continuous Quality Improvement/outcome tools.
9.Link DCF service plans and network provider treatment plans including coordination with foster care reviews, DCF clinical conferences, etc.
10. Write up and record treatment plans for Family Networks families.
11. Respond to crisis management needs of the area DCF office.
12. Submit required documentation in a timely manner.
13. Attend trainings and meetings as required.
14. Comply with all statutes and regulations relevant to the maintenance of clinical standards in the Commonwealth, e.g., mandated reporting, and duty to warn.
Schedule: Monday-Friday 9 AM- 5 PM
Salaary: 40,000 annual
Bachelor's degree in a related field with a minimum of three years clinical/child welfare experience. Experience in permanency planning in a child welfare setting and knowledgeof system of care practices. Experience working with the educational and court systems. Demonstrated sensitivity to the needs of families from diverse cultural and linguistic backgrounds. Computer literacy and skill in the use of computers and related softwareapplications. Ability to consistently meet all necessary background and credentialing requirements. Must have valid drivers license and reliable transportation to and from work and for providing services in the community. Must have reliable telephone notificationsystem for emergency contact. Must be proficient in English and possess and convey excellent verbal and written communication skills with clients, staff and service providers. Demonstrate sound judgment and effective, solution focused, problem solving skills.Ability to develop and maintain professional working relationships with clients, staff, co-workers and supervisor. Ability to work effectively with diverse populations and cultural groups.
