We're The Original Originator

Once upon a long time ago, going out to eat meant either a fancy, pricey, fussy restaurant, or a fast food drive-thru meal handed to you in a greasy paper sack. Our namesake, Norman Brinker, changed all that by pioneering the idea of "casual dining". And he knew an original when he found it.

Guests in Chicago knew Maggiano's Little Italy was special when we opened our doors on the corner of Clark and Grand. Today, thousands of restaurants later, Brinker International is the name behind good food, good times and great places to work. And Maggiano's is our own little slice of Italian American tradition come to life! Tony Soprano would be so proud.

The position performs tasks both in the PACE Day Center and in the PACE Participant's home which support the social, physical and cognitive functioning of seniors with impairments. The Universal Care Partner demonstrates the knowledge and skills necessary to provide an/or assist with appropriate care to include: personal care (feeding, grooming, bathing, toileting), home services to include laundry, memory stimulation activities (social, recreational), motor activities, escorting to medical appointments, shopping, assisting with transportation of participants to and from the facility utilizing the facility transportation vans and other activities as directed by the Participant's plan of care (POC). Assists with any identified needs of the Participant during their onsite stay at the PACE facility and in their home that will enable the Participant to perform at the greatest level of independence possible. Essential Functions and Work Responsibilities Functional Category: Client/Participant Care Estimated Percent of time Spent – 100% * Able to communicate effectively in a positive tone and manner in every interaction. * Integrity * Problem Solving * Action Oriented * Dependability * Arrives at work on time and ready to contribute. Accepts responsibility and works independently to meet deadlines. * Greets and assists participants and their family / caregivers upon arrival to and departure from the facility in a welcoming and caring manner. * Performs nursing care duties as directed by participant's plan of care and/or as directed by the Home Care, Clinic Nurse Managers or by the assigned RN and/or PT to include taking vital signs (blood pressure, weights, temperature and blood sugar at scheduled times) and accurately completing all documentation of results. Also provides assistance with the following: application of lotion, measures intake and output as ordered, obtains stool specimen, empties/changes Foley catheter drainage bag, and applies TED stockings. Compassionately engages participants in activities, conversations, recreational activities, and motor activities in both the facility and home environments to include restorative therapies and activities. Accepts guidance and instruction from members of the PACE Clinical teams and/or management involving Participant care. * Ensures availability of resources (equipment, instruments, and supplies) necessary to ensure positive patient outcomes. Assists with cleaning and maintaining order of assigned patient care areas to include showers, bathrooms, kitchen and activities area so that the facility and / or home environment is clean, safe and supportive for the Participants. Responsible for linen changes in the facility as needed as well as in the homes. * Completes assigned personal care tasks according to Participant's plan of care and/or schedule to include shower/bathing; grooming, toileting and feeding in a thorough manner with the utmost attention to quality and safety of care provided and that maintains the confidentiality of patient/customer information as required by InnovAge PACE policy and HIPAA regulations. * Patient electronic medical record, management observation, patient satisfaction surveys. * Exhibits efficiency, competency, and professionalism that promotes a positive work environment. * Provides a reassuring and seamless experience for customers/patients utilizing Riverside services. * Demonstrates commitment to Safety and Quality. Prevents or reduces the risk of harm and adverse outcomes to our patients, customers, and team members by assuring compliance with RHS safety and quality best practices. * Demonstrates personal accountability of resources by effective time management, reducing expenses (costs), and flexibility in an effort to increase service access to our customers. * Must be able to exhibit competency before performing personal care / aide services; before rendering services to Participants at home, the Universal Care Partner (UCP) must be checked off on a skills check list by a Registered Nurse. UCP will not be able to render any service which he/she has not successfully demonstrated competence and will not be able to render any care if more than one of the core skills are not demonstrated at the appropriate skill level.
