Outpatient Specialty Social Workers are assigned to one or more specialty clinics and/or specialty programs in Departments such as Emergency Medicine, Hospital and Specialty Medicine, Neurology, and/or Operative Care. Services include primary and secondary health care services, to include, but not limited to, psychosocial screening and assessment, psychosocial treatment and intervention, specialized case management and care coordination, discharge and/or after-care planning, health education and advanced health care planning, supportive counseling and caregiver support, and administrative and professional duties. Outpatient Specialty Social Workers work closely with other members of the assigned specialty care interdisciplinary treatment teams to which they are assigned.

Duties Include:

  • The social worker will provide medical, behavioral, and crisis intervention social work services to Veterans presenting in Urgent Care.

  • Require a high degree of clinical knowledge and experience dealing with highly complex health and mental health problems, adjustment to illness and loss, elder abuse, self-neglect, dual diagnosis, intimate partner violence, substance abuse and other presenting issues.

  • The Senior Social Worker has advanced practice skills to provide specialty care for complex, medically compromised veterans, as well as to provide consultation on complex cases and disposition issues to other social workers and to interdisciplinary staff. The Senior Social Worker

  • covers Urgent Care and coordination of the Lodger Program

  • Services include care coordination and consultation with medical, mental health, nursing, and social work staff in other care areas to which Veterans are referred, consulted, and/or also treated and/or co-managed.

  • Provides assessment, evaluation, treatment, and reporting for cases of suspected or reported abuse, neglect, or financial exploitation which present or are identified in the Urgent Care.

  • Works closely with members of the acute psychiatry consult team and suicide prevention program to ensure appropriate assessment, care, and follow up for Veterans experiencing acute psychiatric crisis.

  • Maintains data tracking and workload reporting for Urgent Care social work services provided

Work Schedule: Monday - Friday; 10:30am to 7:00pm

Child Care Subsidy: After 60 days of employment, full time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.66.

Qualifications

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.

Basic Requirements:

Education: Must have a Master's degree in Social Work (MSW) from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. (TRANSCRIPTS REQUIRED)

Licensure: Must be licensed or certified by a state at the advanced practice level at the GS-12 grade level. This includes an advanced generalist or clinical examination, unless they are grandfathered by the state in which they are licensed to practice at the advanced practice level (PROVIDE COPY)

IN ADDITION TO THE BASIC REQUIREMENTS YOU MUST MEET THE FOLLOWING GS-12 GRADE REQUIREMENTS:

EXPERIENCE: must have at least two years of experience post advanced practice clinical licensure and should be in a specialized area of social work practice of which, one year must be equivalent to the GS-11 grade level. Senior social workers have experience that demonstrates possession of advanced practice skills and judgment. Senior social workers are experts in their specialized area of practice.

AND

Applicants must also meet the Demonstrated Knowledge, Skills, and Abilities listed below:

1.Skill in a range of specialized interventions and treatment modalities used in specialty treatment programs or with special patient populations. This includes individual, group, and/or family counseling or psychotherapy and advanced level psychosocial and/or case management.

2.Ability to incorporate complex multiple causation in differential diagnosis and treatment within approved clinical privileges or scope of practice.

3.Knowledge in developing and implementing methods for measuring effectiveness of social work practice and services in the specialty area, utilizing outcome evaluations to improve treatment services and to design system changes.

4.Ability to provide specialized consultation to colleagues and students on the psychosocial treatment of patients in the service delivery area, as well as role modeling effective social work practice skills.

5.Ability to expand clinical knowledge in the social work profession, and to write policies, procedures, and/or practice guidelines pertaining to the service delivery area.
