Summary

The incumbent in this position functions as the Medical Center's Liaison/Case Manager for Homeless Veterans residing in the Farmington area and are part of Albuquerque VAMC's Healthcare for Homeless Veterans Program, Department of Housing and Urban Development / VA Supportive Housing (HUD-VASH). The incumbent is responsible for monitoring all phases of programming and coordination of case management services for Veterans receiving housing provided through HUD Section 8 housing Vouchers.

Responsibilities

The incumbent is responsible for overseeing all aspects of the HUD-VASH program for their assigned team in the Albuquerque, NM catchment area. The incumbent is responsible for reviewing agreements between Section 8, landlords, Public Housing Authorities and the VA in establishing appropriate referral sites for HUD-VASH placements. Additionally, serves as a member of a large Health Care for Homeless Veteran (HCHV) multi-disciplinary team with varied programming to support and enhance medical and psychosocial functioning of assigned Veterans.

The incumbent is responsible for collecting and submitting HUD-VASH Program participant data as outlined by NEPEC evaluation procedures.

The incumbent is responsible for providing oversight of HUD-VASH case management on their assigned team.

The incumbent coordinates care including serving as an advocate on behalf of the Veterans to ensure comprehensive service delivery, and linking and referring Veterans to the VA Medical Center, VA Regional Offices and/or community agencies.

The incumbent complies with criminal conflict of interest laws and Executive Branch Standards of conduct so as to avoid conflicts of interest while serving as the HUD-VASH Case Manager.

The incumbent actively participates in the HUD-VASH Program conference calls and attends training offered by the HUD-VASH.

The incumbent is responsible for developing and administering local policy and SOP for the HUD-VASH Program. This includes maintaining policies and documentation as required for CARF and JCAHO accreditation.

The incumbent actively participates in program planning and development, advising the supervisor of shifting trends with caseloads and identifying the need for and making recommendations on program procedure or policy changes based upon work experiences.

He/She participates in the orientation/teaching of graduate social work/nursing students, students of other disciplines, and Medical Center personnel on the biopsychosocial aspects of homelessness.

The incumbent is responsible for overseeing all aspects of the HUD-VASH program with Veterans served by the Farmington CBOC. The incumbent is responsible for reviewing agreements between Section 8, landlords, Public Housing Authorities and the VA in establishing appropriate referral sites for HUD-VASH placements. This is a mobile position; there is no assigned office space. Instead, most services occur in the community as the incumbent provides intensive case management services to facilitate the provision of housing services.

The incumbent is responsible for establishing a process for referrals and admissions to the HUDVASH Program, ensuring that homeless veterans are considered as primary participants for program acceptance.

He/She performs site visits of HUD-VASH apartments to ensure that veterans reside in safe environments in compliance with local housing codes; verifies the veteran's status, eligibility for VA medical care, and eligibility of program participation.

The incumbent is responsible for providing oversight of HUD-VASH case management. Case management is defined as the provision of services by VA clinical staff to homeless veterans and includes all activities to provide appropriate treatment and maintain veterans in HUD approved stable housing.

He/She coordinates care including serving as an advocate on behalf of the veterans to ensure comprehensive service delivery, and linking and referring veterans to the CBOC, VA Medical Center, VA Regional Offices and/or community agencies.

The incumbent is assigned to Social Work Service and reports directly to the Health Care for Homeless Veterans Program Supervisor. The incumbent is assigned to the VA Medical Center and provides comprehensive social work services to veterans and their families. Responsibilities include the management, coordination and provision of social work services to veteran and their families in inpatient, outpatient and community settings. In the provision of care in patients from the adult (less than 65 years of age) to the geriatric (greater than 65 years of age), the employee recognizes the age related needs and demonstrates knowledge and care appropriate to that age group. In the performance of official duties, the incumbent has regular access to printed and electronic files containing sensitive data which must be protected under provision of the Privacy Act of 1974 and other applicable laws, Federal Regulations, VA statues and policy. Incumbent is responsible for protecting data from unauthorized release of access codes, etc., as stated in the computer access agreement which the incumbent signs.

Qualifications

Basic Requirements:

  • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.

  • Education. Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work.

  • Licensure. Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level.

  • English Language Proficiency. Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. 7403(f).

May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).

Grade Determinations: In addition to the basic requirements for employment, the following criteria must be met when determining the grade of candidates.

Senior Social Worker, GS-12

(1) Experience/Education. The candidate must have at least two years of experience post advanced practice clinical licensure and should be in a specialized area of social work practice of which, one year must be equivalent to the GS-11 grade level. Senior social workers have experience that demonstrates possession of advanced practice skills and judgment. Senior social workers are experts in their specialized area of practice.

(2) Licensure/Certification. Senior social workers must be licensed or certified by a state at the advanced practice level which included an advanced generalist or clinical examination, unless they are grandfathered by the state in which they are licensed to practice at the advanced practice level (except for licenses issued in California, which administers its own clinical examination for advanced practice) and they must be able to provide supervision for licensure.

(3) Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, candidates must demonstrate all of the following KSAs:

(a) Skill in a range of specialized interventions and treatment modalities used in specialty treatment programs or with special patient populations. This includes individual, group, and/or family counseling or psychotherapy and advanced level psychosocial and/or case management.

(b) Ability to incorporate complex multiple causation in differential diagnosis and treatment within approved clinical privileges or scope of practice.

(c) Knowledge in developing and implementing methods for measuring effectiveness of social work practice and services in the specialty area, utilizing outcome evaluations to improve treatment services and to design system changes.

(d) Ability to provide specialized consultation to colleagues and students on the psychosocial treatment of patients in the service delivery area, as well as role modeling effective social work practice skills.

(e) Ability to expand clinical knowledge in the social work profession, and to write policies, procedures, and/or practice guidelines pertaining to the service delivery area.

Physical Requirements:The physical demands involve primarily walking, talking, driving, using the telephone etc. Strenuous physical activity is not required although the incumbent is expected to withstand pressure and stress as part of their day to day activities. This position may also required light lifting under 15 pounds, light carrying under 15 pounds, use of fingers, both hands required, walking up to 4 hours, standing up to 2 hours, climbing - legs only up to one hour, the ability for rapid mental and muscular coordination simultaneously, near vision correctable at 13" to 16" to Jaeger 1 to 4, Far vision correctable in one eye to 20/20 and to 20/40 in the other , use of computer screens, and hearing aid is permitted.
