The Community Alternative Response Team (CART) is seeking two Senior Psychiatric Mental Health Professionals to complete intakes and initial clinical assessments with individuals referred to CART for services. In this position you will also provide case management services as a member of CART's interdisciplinary team.

CART is a short-term, intensive case management team, that focuses on providing voluntary person-centered stabilization services to adults at risk of civil commitment due to having a mental health or substance use disorder. CART works with residents to identify their needs and provides wrap-around support to help them access services in the community. The team is comprised of Senior Psychiatric Social Workers, a Staff Nurse, a Human Services Representative, and a Housing Case Management Assistant.

Location and hours:

These positions are hybrid and will work in Hennepin County community locations and remote as job duties require. Work hours are Monday through Friday from 8:30 a.m. to 5 p.m. While these positions are designated as hybrid, based on current requirements hires must reside in or relocate to Minnesota or Wisconsin.

New employees who are hired into remote or hybrid positions between January 2, 2022, to December 31, 2024, will receive $500 toward the cost of establishing consistent internet connectivity, payable upon completion of 6 months of employment.

In these positions, you will:

  • Provide intensive, clinical services to adults with mental health and/or substance use disorders using a person-centered, trauma informed approach that incorporates the individuals' strengths and identified goals.

  • Complete initial clinical assessments and intakes with individuals referred to CART for services, including collaborating with the Hennepin County Pre-Petition Screening Team to offer CART services to individuals diverted from the civil commitment process.

  • Conduct clinical interviews with individuals to assess needs and work within a multidisciplinary team to connect clients to services and supports in the community. This may include referrals to services for mental health, medical health, basic needs, housing, financial assistance, and employment.

  • Complete intake summaries and functional assessments, develop individual community support plans, and prepare diagnostic assessments as needed.

  • Assess changes in functioning and mental status and respond to changes to meet client's needs in a timely manner and provide crisis intervention when necessary.

  • Coordinate and collaborate with individuals, families, and providers in the community.

  • Engage in clinical consultation, training, and community engagement.

Need to have:

  • Master's degree or higher in social work or a closely related field and two or more years of post-master's degree experience in a psychiatric or mental health setting.

  • Qualification as a Mental Health Professional as defined by the state of Minnesota.

  • Eligibility for or licensure as an Independent Clinical Social Worker (LICSW) by the Minnesota Board of Social Work.

  • Eligible for or licensure as one of the following in the state of Minnesota:

  • Licensed Professional Clinical Counselor (LPCC)

  • Licensed Psychologist (LP)

  • Licensed Marriage and Family Therapist (LMFT)

A valid driver's license, ability to obtain a Hennepin County driver's permit, and access to reliable transportation to visit community sites throughout Hennepin County. Nice to have:

  • Experience:

  • Working with adult mental health targeted case management services and other treatment programs that work with individuals with serious and persistent mental illness.

  • Providing clinical assessment of mental health and substance use disorders and completing intakes.

  • Applying harm reduction and person-centered thinking principles in work or community settings.

Knowledge of:

  • Trauma-informed care principles.

  • Person-centered thinking.

  • County and community resources for adults with mental health and substance use disorders.

Ability to:

  • Establish effective relationships with individuals using a person-centered, trauma informed approach.

  • Function well in unpredictable and challenging situations calling for sensitivity, flexibility, resourcefulness, and independent judgment.

  • Work collaboratively as part of a multi-disciplinary team.

  • Organize information and effectively communicate verbally and in writing.
