Swedish Medical Group is looking for a Senior Patient Care Coordinator (1.0FTE, Day Shift) to work in Sni Neuro Opthalmology at Swedish Cherry Hill in Seattle, WA .

The Senior Patient Care Coordinator is responsible for the overall coordination of seamless care for patients in a surgical and/or non-surgical setting. They will serve as a mentor and resource to Patient Care Coordinators by providing support, guidance and training as needed

In this position you will:

Transmit work instructions from the Supervisor/Manager to the staff, assign workload duties to the staff as approved by the Supervisor, ensure transactions are logged and keyed accurately and on a timely basis providing a smooth flow of work through the department and notify the
Supervisor/Manager of backlogs.
Evaluate the efficiencies of the team as well as identifying barriers and move to eliminate them where possible.
Assist with monitoring performance and relaying feedback to caregivers and supervisors.
Act as a resource for staff members, troubleshoot most difficult problems within the department and find appropriate solutions.
Work closely with Supervisor/Manager to continually update and improve functions.
Assist in planning and implementing new clinic procedures and human resources policies and procedures within the department.

Required qualifications for this position include:

High School diploma or GED
2 years experience in healthcare setting with increased responsibilities or 1 year experience in Patient Care Coordinator type role.

Providence St. Joseph has established an ambulatory care division designed and organized to focus on providing patients access to an optimized and connected ambulatory care network across all our geographies. This lower cost, consumer-centric network is a key strategy to ensure we are the preferred health partner in the communities we serve. The Ambulatory Network includes assets in each of our markets for the six key clinical pillars: * Retail * Primary Care * Urgent Care * Ambulatory Surgery * Behavioral Health * Freestanding Diagnostic Imaging In this position you will have the following responsibilities: * Serve as a transformational member of the Ambulatory Care leadership team; working collaboratively to achieve strategic and operational goals as outlined in the integrated strategic and financial plan and as directed by the Executive Director-Ambulatory Care Network. * Conduct strategic assessments and analysis. In partnership with business leaders, creates business cases and business plans for initiatives. Develops initiative-specific implementation plans and manages activities against the plans. Set up and lead multi-disciplinary work teams. Ensure effective communication, training, and post-implementation support strategies are created and implemented among all stakeholder groups. * Lead development of senior executive chartered projects/initiatives from concept phase through transition to operations using combination of analytical, project management, change management, facilitation, and business management expertise. Ensure tracking of financial, human, and technical objectives for each project. Provide project status reporting to executive sponsor, initiative owner, project team, and other stakeholders as requested. Develop project-specific escalation and issue management protocols. * Provide project leadership as required for large transactions / affiliations and related business development efforts including sourcing and evaluating potential companies to meet needs, value proposition development, business plan development and analysis, financial and business due diligence, project management and internal review processes, formulate and respond to RFPs and support negotiation and review of deal documents including letter of intents, purchase agreements, operating agreements, management services agreements, or other agreements related to acquisitions, joint ventures and divestitures.. * Engage stakeholders in solving problems. Utilize change management and performance improvement tools. Build awareness and commitment to the change effort, and leads the ongoing communications and alignment of stakeholders throughout the process. Conduct post-initiative assessments and makes recommendations for improvement based on those assessment. * Create and maintain governance systems and structures in partnership with Director, Growth and Governance, including charters, agendas, minutes, and tracking documents. Ensure clinical pillar governance and management councils are aligned with overall Ambulatory Care network governance and PJSH governance. Manage frequent and routine reporting and presentations to PSJH Board, Councils, Regional Leadership, etc. * Navigate relationships at all levels within a complex, relationship-based organization; utilizing direct and indirect influence to improve alignment, management, and performance of assets. * Oversee development of consistent and actionable strategic analysis that drives improvement in performance and value to the consumer. * Coordinate activities with physicians, third party payors, staff, consumers and others on a regular basis to assure that work flows efficiently and effectively; and key stakeholders are satisfied with performance and services. * Represent Providence St. Joseph Health, the EVP-Ambulatory Care, and the Ambulatory Network Division to governing boards, senior executives and professional groups both within and outside the organization. Required qualifications for this position include: * Bachelor's degree in business, health care or a related field. * A minimum of seven (7) years healthcare experience, preferably in roles representing a broad range of experience across virtual, retail, ambulatory, inpatient delivery and health plans. * A minimum of five (5) years' experience working as part of a large, multi-product/multi-market, geographically dispersed organization. * A valid driver's license and ability to pass state/federal screening requirements for airline travel. * Experience related to not-for-profit and religious-based organizations is desirable. * Experience in process improvement using Lean methods including visual management boards and department huddles or stand ups. Preferred qualifications for this position include: * A master's degree or equivalent. * A minimum of three (3) years' experience working in collaboration with physicians to achieve demonstrated success in achieving strategic goals. * A minimum of three (3) years' experience in business development, mergers and acquistions, and experience in clinical or healthcare setting. * Lean/ Six Sigma training * Change Management skills About the department you will serve.
