This position performs functions relevant to the documentation of the hospital patients' births and deaths. Collects the necessary data from new mothers and the medical record on a daily basis to complete a birth certificate and footprint certificate for each live birth or to complete a fetal death certificate for each stillbirth.
Mails completed birth and stillborn certificates to the Bureau of Vital Statistics. Completes and processes paternity forms according to established guidelines. Processes death certificates for physician signature according to Bureau of Vital Statistics guidelines.
As needed, mails original death certificates to Funeral Homes following completion by physicians. Compiles a monthly vital statistics report following guidelines established by the State of Alabama Bureau of Vital Statistics and mails report to the Bureau in a timely manner according to established guidelines. Coordinates the activities of staff providing coverage for the position, by distributing work assignments.
Monitors the completion of these assignments and informs the staff of any changes in the established guidelines.
As needed, scans and indexes documents into the electronic medical record to the appropriate encounter number and document name/type with the highest level of quality and while maintaining established turnaround times. Reviews scanned images with an accuracy rate of at least 98%. Inserts and/or appends pages or documents when and where appropriate. Participates as a team member by performing or assisting with other related tasks and duties, as required and assigned by the Operations Manager or HIM Director, to ensure efficiency in department operations, achieve goals in quality outcomes, performance improvement standards and customer satisfaction.
High school diploma or equivalent. Two to three years experience as a Medical Record Analyst in the Health Information Management.
