Senior Habilitation CoordinatorLocation: Montgomery County, NY FLSA: Non-Exempt Reports To: Assistant Director Physical Requirements: Lifting: Ability to lift and transfer consumers, bending, pulling, pushing, walking, and pushing wheelchairs. Hours of Work: Full Time 37.5 hours/wk. Flexible schedule required to provide necessary support. Pay Range: The pay range is $24.25 to $35.26Job Summary: Oversees Day Support Programs. Responsible for all aspects of scheduling, supervision, training; Habilitation plan oversight and fiscal management. Job Qualifications:
Bachelor’s Degree in Human Services, related field or enrollment in an accredited degree program preferred. High School diploma/GED required.
Minimum two years of experience working with individuals with developmental disabilities and/or related human services program required.
Must be willing to get medication administration certified.
Minimum one year experience supervising staff.
Must be able to read and write the English language.
Must have valid NYS Driver’s License.
Travel is required; employee must have reliable transportation available, as Habilitation Coordinators may be managing more than one program site.
Major Responsibilities:
Responsible for the supervision, discipline, hiring and termination of direct support staff.
Responsible for program development and oversight built around individual’s personal outcomes.
Comply with day habilitation regulations, agency P & P, training, and documentation.
Monitor all functions of day habilitation operation; ensure supports are present for all individuals.
Meet with subordinates and supervisor as needed for leadership development.
Responsible for assisting the day program scheduler with scheduling needs for assigned program sites as needed.
Perform direct support duties as needed, this varies and is dependent upon site assignments.
Will work with families, advocates, or correspondents.
Ensure that direct reports complete all required trainings.
Responsible for completing and reviewing yearly evaluations and leadership development tools.
Responsible for managing budgeted FTE’s and OT.
