Position Summary

The Senior Community Liaison is responsible for coordinating all programs and activities targeting the senior population served by Saddleback Memorial Medical Center, as well as initiating and maintaining strong relationships between community leaders.

This position requires the full understanding and active participation in fulfilling the mission of Saddleback Medical Center. The employee shall support Saddleback Medical Center's strategic plan and participate in and advocate performance improvement/patient safety activities.

Essential Functions and Responsibilities of the Job

  • Coordinates a minimum of 16 educational/outreach events

  • Initiates and maintains relationships between SMC and Senior facilities in the area. Acts as liaison to Laguna Woods Senior Residents population, Clubs and Village Management Services Leadership.

  • Navigates Seniors and family members in person, virtually, or via phone.

  • Utilizes critical thinking skills to facilitate seamless navigation of patient's care cycle effectively and consistently.

  • Facilitates patient and family member feedback and satisfaction results and implements real time plan of action to address any issues identified in coordination with multidisciplinary teams.

  • Coordinates special events and educational programs for physicians and the senior community.

  • Assists in the writing, development and printing of promotional materials related to senior programs and activities.

  • Initiates and maintains relationships between SMC and senior communities, centers and health care facilities in the area.

  • Attends and supports other civic, community and service organizations' meetings and events.

  • Supports the work of the SMC Foundation through community event participation.

  • Ability to prioritize and multitask.

  • Other duties as assigned.

  • Placement in the pay range is based on multiple factors including, but not limited to, relevant years of experience and qualifications. Minimum Requirements

Qualifications/Work Experience:

  • Has experience and passion for working with the senior community

  • Strong background in community involvement and initiatives

  • Ability to organize projects and enlist needed support for completion

  • Proficiency in Microsoft Office

  • Knowledge of Medicare and Medigap insurance

  • This position requires a highly organized person with excellent communication and customer service skills, attention to detail, time management and consistent follow-through.

Education/Licensure/Certification:

  • Bachelor's degree in health or related field required.

  • Master's degree in health or related field preferred.
