Position Summary:

We have an exciting opportunity to join our team as a Senior Community Health Representative.

The Senior Community Health Representative will provide culturally-tailored outreach, health education and/or individual counseling relevant to the Program/Project, maintaining a complex caseload. Works under the supervision of the study Principal Investigator and Study Manager.

Job Responsibilities:

  • Navigate patients to health and social support services, maintaining a caseload that may be higher in volume or include patients with more complex needs

  • Provide health education and/or individual counseling through culturally-tailored health education workshops and trainings

  • Conduct community and/or patient outreach by attending and/or coordinating health fairs, community forums, etc.

  • Assist in the creation of culturally specific handouts, flyers, and other materials related to the program or study

  • Develop new and foster existing relationships with community and/or patient population to support enrollment Conduct trainings and/or talks for other Community Health Representatives and internal or external audiences

  • Conduct intake or interviews/surveys with patients; keep participants motivated and engaged in the program or study

  • Keep organized patient material, including logs and data files, in order to track patient outcomes

  • Attend trainings as required by NYULH, funding agency, and/or research study team

  • Work as part of a healthcare team

  • Performs other duties assigned

Minimum Qualifications:

To qualify you must have a High School Diploma, GED, or equivalent. Minimum 1 year of direct client/patient service experience in community settings.

Effective oral, written communication, and interpersonal skills. Empathetic, reliable, resourceful, and strong desire to help people.

Preferred Qualifications:

Minimum 3 years community health worker experience. Demonstrated success in reaching caseload and/or recruitment targets.

Demonstrated success in patient/community outreach. Interest in providing trainings/talks for other community health worker and internal/external audiences. Proficient with reporting tools (e.g., Excel, REDCap, salesforce, etc.). Fluency in [preferred language]. Familiarity with [preferred] community-based organizations, community leaders, and community resources.

Available to work evenings and weekends. Available to travel throughout Manhattan, Queens, Brooklyn, and Bronx. Ability to juggle priorities and manage time; work well under pressure.

Good advocacy and community organizing skills. Basic computer knowledge/skills required. Strong organizational skills. Works well with a wide range of people.

Qualified candidates must be able to effectively communicate with all levels of the organization.

NYU Langone Health provides a salary range to comply with the New York state Law on Salary Transparency in Job Advertisements. The salary range for the role is $48,274.10 - $59,600.93 Annually.

Actual salaries depend on a variety of factors, including experience, specialty, education, and hospital need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
