JOB SUMMARY:

The Senior Clinical Director of TruHealth will develop, implement, and manage clinical care initiatives. This position will have direct and indirect oversight responsibility for nurse practitioners, physician assistants, RN case managers and administrative staff. The Senior Clinical Director will also be responsible for ensuring that all staff have the training and tools to provide members, patients, and family members excellent service and a level of care that improves the quality of life of the patients served. Work to support the VP of TruHealth with training, developing and coaching teams.

ESSENTIAL JOB DUTIES:

  • Focus to critically shape the transition from a traditional model of care to a focus on disease prevention, management of chronic illnesses, and primary care for a broader Medicare Advantage "Institutionalized" Special Needs Population throughout the U.S.

  • Focus on providing information and care coordination within the total spectrum of care management services

  • Operate in a collaborative interdisciplinary team environment

  • Emphasize the interaction and communication among patients, healthcare providers, case managers and other allied health professionals

  • Ensure coordination of care and appropriate cost-effective services are provided by the clinical team

  • Other duties as assigned

REQUIRED QUALIFICATIONS:

  • Experience:

o Minimum of three (3) years' experience in leading/managing a clinical team

o Five (5) years of experience in long term care or chronically ill patients

o Strong background and familiarity with Medicare, Medicaid, ISNP, nursing facility and managed care preferred

o Strong working knowledge of HEDIS/ STARs preferred

  • License/Certification/Education:

o Progressive Registered Nurse or Mid-Level Practitioner

o Graduate from an accredited school as a Registered Nurse required

o Graduate from an accredited school as a Mid-Level Practitioner preferred

o Current Registered Nurse or Mid-Level Practitioner license within practicing state(s)

o Current CPR certification

o Current state driver's license

o Current automobile liability insurance according to company policy

SUPERVISORY RESPONSIBILITES:

  • Accomplish department objectives by supervising staff; establish goals for performance; set deadlines in compliance with company's plans and vision

  • Develop personal growth opportunities; provide constructive feedback and coaching

  • Organize workflow; ensure employees understand their duties or delegated tasks

  • Monitor employee productivity; provide constructive feedback and coaching
