Job Description
Basic Purpose
Positions in this job family provide professional case management services to assist clients receiving treatment through advocacy, referral assistance, and by preparing individual care plans based on recovery goals and strategies.
Typical Functions
The functions performed in this job family will vary by level, unit and organization, but may include the following:
Provides screening to determine the need for case management services
Develops a strengths-based assessment in conjunction with the consumer
Provides emergency services and emergency assistance
Provides client support and training to maintain community living skills
Networks with other individuals and organizations that influence the recipient's relationship with the community (e.g., family members, law
enforcement personnel, landlords, etc.)
scheduled appointments (including physician/medication, counseling, r
rehabilitation, or other supportive service appointments as delineated on the service plan)
Documents services and maintains necessary records
Meets billable hours for Certified Community Behavioral Health Clinic (CCBHC) facilities
Level Descriptor
Levels are distinguished based on the complexity of assigned duties, the level of expertise required for completion of work assignments, and the responsibility assigned for the supervision of others.
This is the specialist level of the job family where incumbents are responsible for performing a full range of professional case management activities. This position may be assigned as a lead worker in a team environment. Incumbents may provide training or assistance to lower-level employees. Incumbents are responsible for the safety of the patients under their care.
Knowledge, Skills, Abilities and Competencies
Knowledge, skills, and abilities required include knowledge of the principles and practices of case management techniques; consumer-driven services; strengths-based principles.
Skills required include the application of active listening and problem solving for consumers receiving services for mental health, substance abuse, and co-occurring disorders; clear communication, both written and oral; and team building.
Abilities required include working in a high-stress, and fast-paced environment.
Education and Experience
Education and Experience requirements at this level consist of a bachelor's degree in Education or a behavioral health related field and two years of professional experience in professional case management behavioral health services; or a currently licensed registered nurse in Oklahoma with experience in behavioral health care and two years of professional experience in professional case management behavioral health services; or a bachelor's degree and current certification for Certified Psychiatric Rehabilitation Practitioner or Certified Child and Family Resiliency Practitioner from the United States Psychiatric Rehabilitation Association (USPRA) and two years of professional experience in professional case management behavioral health services.
Special Requirements
Incumbents must function in accordance with the Joint Commission, Commission on Accreditation of Rehabilitation Facilities (CARF), Health Insurance Portability and Accountability Act (HIPAA), and all other applicable rules, laws, and authorities.
Incumbents will adhere to the Professional Standards for Certified Behavioral Health Case Manage (OAC 450:50-7 Rules of Professional Conduct)
Certification as a Certified Behavioral Case Manager will be completed within 30 days of start date with the agency, contingent to training availability.
