What You'll Do:

  • Create a welcoming environment for our guests by ensuring homes are cared for and cleaned to Vacasa standard

  • Note any damages to homes and create maintenance tickets as necessary

  • Replenish consumable items such as soaps and paper products

  • Maintain hot tubs; no experience necessary - we offer on-site training

The Skills You'll Need:

  • Reliable transportation

  • Weekend availability

  • Attention to detail

  • Housekeeping experience, although we can teach you the Vacasa standard

  • Ability to stand for extended periods of time, bend, stoop, squat, kneel and lift over 20 pounds

  • Comfortable working with computers and the Internet


  • $15/hr

  • Seasonal pay increase from June 2 to September 7, 2019 to $17.00, after this period pay will revert back to $15/hr

The Population Health department is integrally linked to payer contracting, care coordination in the inpatient and outpatient setting, local and regional health plans, community needs and integration, public health and regional non-profit entities, and data analytics. POSITION OVERVIEW: The Nurse Care Coordinator Ambulatory Care, in collaboration with the comprehensive primary care team, coordinates and provides health services for a predefined population of patients in alignment with St. Charles Medical Group clinics and St. Charles organizational goals. Through effective partnerships with patients, their caregivers/families and community resources, the Nurse Care Coordinator Ambulatory Care facilitates patient-centered care planning driven through evidence based practice protocols by engaging in health visits (both in clinics and in patient's homes), preventive screenings, patient education and chronic disease management within the scope of the RN's training and licensure. This position does not directly supervise any other caregivers. ESSENTIAL FUNCTIONS AND DUTIES Responsible for the care coordination of patients at the St. Charles Medical Group clinics who have high-risk conditions and/or are in need of chronic disease management through early detection and ongoing monitoring or intervention. Responsible for health visits successful post-acute care transitions as evident by predefined performance measures. Responsible for the successful completion of the pre-determined quality measures for assigned patient panels. Responsible for health visits pertaining to annual wellness for the Medicare population to assist in the organizational ACO requirements under the scope of the RN's training and licensure. Conducts home visits with patients, as appropriate, to provide comprehensive care services to the most at risk patient population. Acts as a lead in the development and documentation of the patient/family-centered plan of car in collaboration with the care team. Monitors adherence to plan of care and facilitate changes as needed. Supports primary care team with timely communication, inquiry, follow-up and integration of information into the care plan regarding transitions of care and referrals. Cultivates effective partnerships and works collaboratively with all members of the care team, including the patient and family. Maintains understanding of and conducts patient education for a wide scope of health care conditions and diseases with the goal of moving patient towards effective self-management. Participates in creating intentional relationships and demonstrates positive attitudes and behaviors that enhance the care experience. Develops and maintains knowledge of federal and state regulations, basic insurance knowledge, benefits, services and limitations. Maintains knowledge of all metrics and outcome measures that Population Health is at risk for and performs all tasks to meet those requirements. Attends and engages in weekly clinical care team huddles to discuss population health level data. Functions as an active liaison between the clinics, hospital and other care settings and agencies throughout the community. Collaborates and coordinates with the in-patient Case Management and Nurse Navigation teams to streamline the transitions of care from the in-patient to outpatient clinic environments. Attends and actively participates in population health related trainings and meeting activities as scheduled. Participates in creating a healing environment that supports all aspects of the care environment and the wholeness of each individual, patient and caregiver. Provides a therapeutic presence in service to others by purposefully responding to the needs of patients/customers in a caring way. This includes introducing oneself and stating your purpose, asking the patient their preferred name, sitting with the patient to determine their care goals, active listening, and the skillful use of communication skills (touch, eye contact, etc.). Provides and maintains a safe environment for caregivers, patients and guests. Documents all patient care with proficiency in compliance with hospital policies and regulatory agencies Engages in functions (capture/monitor data, health visits, etc.) to provide support in meeting reporting requirements of identified contractual obligations (ACO, PCPCH, CPC+, etc.).
