Responsible to perform all Supply Distribution Services functions. Performs inventory management.

Delivers a high level of customer service while ensuring the satisfaction of our internal customers. Participates in providing and maintaining accurate information and inventory levels for our customers and staff. Uses technology to its fullest potential.

Ability to drive a large medical equipment and supply transport truck as well as courier functions when needed. 2 years# experiencein warehouse and or/distribution/logistics. Previous healthcare and medical supplies experience preferred. HS Diploma or equivalent required.# Current Valid PA Driver#s License with no incidents required.

Must be able to drive large equipment and supply truck transport with lift gate operations as well as courier duties and receiving functions. Must possess the ability to maneuver weights in excess of 100 pounds. # Basic computer/PC navigation skills, warehousing devices and equipment, hardware and software. # #

Acts as a coordinator for hospitalized patients with complex learning needs regarding diabetes self-care: promoting a safe transition from hospital to the community. Provides suggestions to referring providers for pharmacological and non-pharmacological interventions, focusing on patient centered care. Serves as an expert in principles of management of the hospitalized diabetes patient and contributes to the development of quality improvement systems promoting change in organizational behavior. Utilizes evidence based interventions to assist in achieving patient glycemic targets and improve patient outcomes, resulting in decreased length of stay. Minimum Requirements: Bachelor#s degree in Nursing or related field required. Two years background in clinical patient education. One to two years recent experience in direct diabetes patient education. Must be able to creatively problem solve, function in a self-directed work team environment, work independently with minimal supervision, prioritize workload and demonstrate critical thinking skills. Specializes in the application of education and behavioral change theories. Must be able and willing to travel between multiple hospital and regional locations to provide educational service. Ability to assess social, economic, and cultural influences impacting the patient#s ability to self-manage diabetes. Preferred Qualifications: Master#s degree in Nursing or related field preferred. Three to five years experience in direct diabetes education. Two or more years in an inpatient setting.
