Under general supervision and according to Service's standards and policies, prepares social assessments, and functions as the clinics primary mental health provider at the school tele-health clinics. The Social Worker will act to restore students to optimum social functioning within their capacity. The Licensed Masters Social Worker confers with clinic providers, nurses, and works directly with students and families. The CLMSW performs mental health functions independently within determined scope of practice for the Community Health Center of Branch County, Michigan Mental Health and Public Health Codes. The social worker has the ability to conduct crisis and emergency interventions.
Demonstrates proficient observational and assessment skills in developing plans and treatments according to social work theories and practices
Participates in assigned patient care as assigned and meets established productivity standards.
Utilizes knowledge of social work processes and skills.
Provides direct social work services using various modalities including, but not limited to psychotherapy, casework, and crisis intervention.
Coordinates mental/emotional treatment sources, and advocates on behalf of patients and families.
Conducts crisis/emergency intervention Identifies social problems, their severity, inter-relatedness to the medical situation, ability of family to cope with situation, and discusses alternate methods of alleviating the situation.
Evaluates medical social worker needs of the patient, utilizing the appropriate assessment tools.
Observes, evaluates, records, and reports to the physician and others, as needed, the patient's physical, emotional, social, spiritual, financial condition and the patient's reactions to drugs and treatments.
Develops a social work treatment plan, utilizing information from patient, family, agency personnel and others appropriate to adolescents. Develops and implements an appropriate social work care plan by information gathering, interviewing, and considering various treatment methods. Initiates and participates in case conferences.
Sets priorities for his/her case management. Performs efficient therapy as a singular therapist
Develops specific and timely case closure/after care plans Demonstrates knowledge of age-specific developmental factors necessary to provide clinical therapy.
Completes all necessary paper work thoroughly, accurately, and in a timely manner, as specified by the Service's operating procedure, also defined by insurers, HIPPA, MDHHS, NASW standards for clinical social work, MI Mental and Public Health Codes, and best practice.
Master's degree from an approved school of social work (accredited by the Council of Social Work Education)
One year of social work in a health care setting.
Preferred experience with adolescent population and families.
