School Based Services
Establishes leadership and support in the program.
Provides counseling services to individuals, groups and family, case management, respite services and crisis intervention.
Works with identified students across the school or school district.
Works collaboratively within an interdisciplinary team to ensure effective establishment and maintenance of the behavior management program.
Provides support to students, teachers, staff members, and parents for the purpose of maintaining consistency in care.
Communicates observations to supervisor or designee.
Provides interventions and information to the classroom staff and parents, as assigned.
Provides crisis intervention and supportive interventions, including case management, linkage to community resources, and provides care coordination of service delivery.
Assists in increasing success in the overall objective of the program.
Facilitates school or district wide risk assessments (clearances), as needed.
Completes all documentation (e.g. comprehensive assessments, treatment plans, clearances, therapy, and contact notes) in the time frame required as per policy. Documents services on a timely basis with all entries authenticated and time/dated.
