The Patient Care Coordinator works closely with Nurse Navigators to ensure appropriate post-discharge patient follow-up. S/he is responsible for organizing and grouping patient appointment needs by individual physician practices to facilitate efficient communication with office staff. Weekly communication with office staff includes calling to make appointments and calling to confirm appointments. S/he is responsible for rescheduling missed appointments and then coordinating communication back to Care Assure Nurse Navigators for patient follow-up. All activity is logged in the web-based tool and communicated back to the Care Assure team.
This job is a Monday to Friday, 5 days-per-week position. No weekends or holidays required.
Schedules follow-up appointments and provides information with reminders to patients & physician offices
Tracks outcomes of referrals and treatments in web-based tool and excel (as needed)
Maintains detailed records to measure effectiveness of program, including quality check of dashboard/applications and suggests ways to improve throughout the process
Communicates appointment planning with patients and families
Answers patient/family phone calls, fields' questions appropriately
Participates in creating an environment that implements the mission, vision and values of the organization and Care Assure program
Graduate of an accredited school of nursing
Current State of Florida RN license, or Recognized Compact Licensure (subject to Florida State Licensing Requirements, including/not limited to ongoing eligibility and duration provisions)
Current BLS certification via American Heart Association
3 or more years' experience as an RN in an Acute Hospital environment
Skills required: MS Office, hospital based computing systems (i.e. Meditech), critical thinking, excellent interpersonal communications skills, PC Skills
Ability to travel to between Division hospitals in Palm Beach and Treasure Coast markets
