The Hospice CNA participates in stewarding the art of compassionate care through hands-on patient care at the end of patients and families through emergent care visits.

Position

Qualifications:

  • High school diploma or G.E.D. required.
  • Must be certified by the state as a nurses aide with an unrestricted, active aide certification.
  • Three (3) to six (6) months of current experience as a Hospice Aide or Nurses Aide required.
  • Hospice or Home Health Agency experience preferred.
  • Must possess interpersonal skills sufficient to interact effectively with patients and their families, as well as all interdisciplinary caregivers, peers, subordinates, and supervisors.
  • Must exhibit or demonstrate the ability to learn computer skills related to required clinical software and electronic clinical record utilization.
  • Must have a reliable car for transportation, valid drivers license, required automobile insurance, and the ability to safely and independently operate said vehicle.

Shift :

  • Saturday 8:00 PM to 8:00 AM

This position requires population specific competencies. Adheres to National Patient Safety Goals as appropriate based on the level of patient contact this position requires. Essential Functions: The Case Manager Hospice responsibilities include, but are not limited to, the following: * Manages/coordinates all aspects of service delivery as defined by physician orders * Participates in interdisciplinary group meetings/re-certification process. The Case Manager is directly responsible for all aspects of the patient's care. The plan of care is discussed with all the disciplines and reevaluated as needed. This includes needs such as comfort measures, pain management, psychosocial needs, spiritual needs, volunteer needs and/or bereavement issues * Ensures implementation of activities for improved quality of patient care * Supervises Home Health Aide activities and plan of care * Supervises and educates nursing/medical students during their rotation * Specializes in pain and symptom management issues in patient care Accountability: The Case Manager Hospice reports to Manager Hospice.
