At Salt Lake County Housing and Community Development (HCD) we support individuals and families to be healthy, live in healthy places, and have expanded opportunities by responsibly and responsively deploying resources to organizations in Salt Lake County. HCD has been in the business of serving and impacting the lives of people who are experiencing poverty for over 35 years.

Salt Lake County HCD is seeking VISTA members to be part of our team as we develop and implement the "System Navigator" Pilot Program. The System Navigator program is a new initiative by Salt Lake County which will leverage the case management that is provided within individual social service programs, by giving client beneficiaries access to and control of their own client record, allowing them to "own their own story". The game-changer of this program is that Salt Lake County will facilitate the technology that will allow the beneficiaries of human services resources to connect their own data, goals, and documents to the applications, services and programs that they trust. It is believed that the tool will help pave many pathways out of poverty, making the journey smoother and faster.

Successful implementation will also provide meaningful data for stakeholders related to the effectiveness of interventions, which will generate the information needed for data driven decision making. The time and talent of VISTA volunteers that is dedicated to informing the development of this pivotal tool will empower recipients of social services to take charge of their own journey towards self-reliance. VISTA's will have the opportunity to significantly contribute to a permanent change in the way that people in poverty are able to engage in the services which are available to them. VISTA members will be part of the core team that conducts the groundwork and lays the foundation for the system, as well as implements, operationalizes and evaluates it during the critical first three years of development.

VIEW JOBS 4/21/2019 12:00:00 AM 2019-07-20T00:00 OnSite Care is looking for an exceptional FLOAT Medical Assistant to join our team in our team! This will be for our South Salt Lake Clinic. This is a FLOAT position; we can work you up to 40 hours. This position will work both front and back office. For the most part, your time will be spent in the following ways: * You'll be the support for a one provider practice. * You'll keep rooms neat and clean, and prepare rooms for the visits, procedures and patient exams, ensuring the provider has the equipment necessary for the procedure type, and patients are prepped appropriately. * You'll get to know the provider's preferences and actively communicate with the provider to ensure you are providing the level of support necessary for the provider to effectively treat and care for patients. * You'll check patients in and gather the necessary paperwork and prepare patients to be seen by the provider by obtaining vital signs and gathering the necessary information from patients for their visit. Information will be documented into the EMR, which may include the reason for the visit, documenting their vital signs, updating their medications and doses, review and updating their allergies, gathering any past medical and surgical history, and reviewing any updates to family and social history. * You'll obtain patient's vital signs, complete any point of care testing required for the visit type, and ensure patients receive the highest level of customer service possible. * You'll act as a chaperone for your provider during patient exams as requested and assist with any procedure or exam requested by your provider. * You'll perform point of care testing, administer vaccines and injections, obtain ECG/EKGs, and perform phlebotomy to obtain lab samples ordered by provider. * You'll follow up on lab work, act as a liaison between patient and provider, provide patients with prompt responses to inquiries, and complete tasks in a timely fashion. * Most importantly, you'll provide patients with an exceptional experience at every opportunity! What you'll have to be successful in this role: * Successful completion of a Medical Assisting program or equivalent experience. * Experience performing phlebotomy and giving children injections. * Working with patients and customers and have experience providing customer service. * You will be passionate about working in healthcare and want to work for a company that is making healthcare affordable and accessible for patients. * You will be dependable, accountable, and courteous and value quality and continued improvement in everything we do. * You will be a team player, and value the contributions of each person on a team. In a perfect world, you will already have: (Preferred areas - We can train you in these areas) * You will have 1 to 2 years of experience working in an outpatient clinic setting. * Experience working in a Family Practice and/or Pediatrics. * Experience working with an EMR (eCW). * Bilingual (English and Spanish) is definitely a bonus!
