Saint Joseph PACE (Programs of All-inclusive Care for the Elderly) program provides care and services to seniors with chronic care needs allowing them to maintain their independence in their homes for as long as possible. The program offers a full range of health care services for individuals who are eligible for nursing home care but want to live at home. Saint Joseph PACE believes it is better for seniors with chronic care needs to be served in their home whenever possible. Many PACE participants get most of their care from staff employed by the PACE organization in the PACE center. PACE centers meet State and Federal safety requirements and include adult day programs, medical clinics, activities, and occupational and physical therapy facilities.

General responsibilities will include:

  • The Certified Nurse Aide is responsible for providing various care services necessary for attending to the personal needs, support, and health status of participants. This position reports to the Home Care Manager.

  • Participate as a member of the interdisciplinary team in ongoing participant assessment and monitoring. Report on personal care services provided and the self-care abilities of participant, including changes noted in participant's condition.

  • As part of the health care team, provide personal care assistance that will maintain the independence of the participant to conduct activities of daily living, including toileting, showers, eating, and grooming. Assist in the maintenance of physical activity, exercise, and walking.

  • Provide behavioral supervision or support for participants, such as reality orientation, reminder to follow through with nursing and medication instruction, diet restrictions, and emotional reassurance.

  • Assist in identifying signs and symptoms indicating physical, emotional, or behavioral change in participants and reporting these signs to the center staff and/or supervisor, including but not limited to, any changes in skin integrity, bowel and bladder status, food/fluid intake, or changes in ambulation.

Candidates will have:

  • Must be an active certified nurse assistant in the state of Indiana.

  • Must be a High School graduate or equivalent.

  • Valid Driver's License and availability of a vehicle.

  • CPR Certified or ability to obtain within 90 days of hire.

  • Prefer a minimum of one (1) year of documented experience working with a frail or elderly population.

  • Experience in a long-term care facility or with a community-based geriatric program preferred

  • Candidates should possess the ability to work as part of a team, coach and train staff, as well as strong analytical and problem-solving skills.

  • Must possess strong interpersonal skills in order to interact in extremely delicate, sensitive and/or complex situations.

  • Candidates should have excellent communication skills, with the ability to communicate with multiple levels in the organization.

Saint Joseph PACE believes it is better for seniors with chronic care needs to be served in their home whenever possible.� Many PACE participants get most of their care from staff employed by the PACE organization in the PACE center. �PACE centers meet State and Federal safety requirements and include adult day programs, medical clinics, activities, and occupational and physical therapy facilities.� General responsibilities will include: * Demonstrates nursing care to and instructs enrollees and families in treatments and other self-care interventions. * Participates in the development and revision of the plan of care working in collaboration with the interdisciplinary team; maintains timely and accurate documentation of services. * Assesses health needs and contribute to the plan of care, initial implementation of nursing care plans and providing nursing care as well as periodic re-evaluation of individual and family nursing needs. * Participate as a member of the interdisciplinary team (IDT) and in initial, semi-annual, unscheduled, and annual assessments; attend morning updates and report changes in participants' baseline status to appropriate staff daily. * Assure that documentation for the clinic is accurate, current, and that the clinic documentation is up to date. Candidates will have: * Must be a graduate of an accredited Nursing Degree program and possess a current, unencumbered, active license to practice as a Registered Nurse in the state of Indiana. BSN preferred. * Minimum of two (2) years of demonstrated successful experience in related services with geriatric population. * Active CPR certification or ability to obtain within 90 days of service. * Valid driver's license in good standing and proof of insurance required. * Must have the ability to perform standard clinical procedures according to established protocols and possess initiative, solid problem-solving skills and a high level of accuracy and thoroughness in documenting the delivery of services. * Experience in the provision of nursing care, including assessment skills, health education, and current nursing treatments and protocols solid problem-solving skills and a high level of accuracy and thoroughness in documenting the delivery of services. * Must possess strong interpersonal skills in order to interact in extremely delicate, sensitive and/or complex situations. * Candidates should have excellent communication skills, with the ability to communicate with multiple levels in the organization. * Must possess strong analytical and organizational skills.
