Job Responsibilities
The Wraparound Facilitator will utilize a team -based planning process to provide individualized, coordinated, and family -driven care. Foster interprofessional collaboration to provide adaptive and culturally responsive service options to meet the critical and complex needs of those who visit the Child and Family Engagement Center. The Wraparound Facilitator will adhere to Wraparound's 10 value-based principles which include collaboration, cultural humility, family voice and choice, and unconditional care from community-and strengths-based perspectives. The Wraparound Facilitator will embody a commitment to team development, family empowerment, and outcome -based interventions.
The Wraparound Facilitator will provide assistance to address identified child and family needs, to include connection to services.
The Wraparound Facilitator will perform other related duties as assigned, including but not limited to, determining appropriateness of referrals, contacting services on behalf of families.

Minimum and Additional Requirements
A bachelor's degree in guidance, counseling, or the social sciences and professional experience in human services or social services program.
Preferred Qualifications
A bachelor's degree in guidance, counseling, or the social sciences and professional experience in human services or social services program.
Bilingual abilities in English and Spanish (or another language) are a plus.
